The effect of different magnetic field intensities on regeneration at injured peripheral rat nerves [Degişik şiddetteki manyetik alanlarin hasarlanmiş siçan periferik sinir rejenerasyonu üzerine etkileri]

Objective: We investigated the effects of different intensities of sinusoidal magnetic fields of daily life exposures on the peripheral nerve regeneration daily life exposure. The effects of magnetic field (50 Hz frequency and 1, 2, 3 mT intensities) treatment on sciatic nerve regeneration were studied in a model of crush injury of the sciatic nerve of rats. Material and Methods: Prior to crush injury, compound muscle action potentials were recorded for all rats. Then, the sciatic nerve was compressed for 15 sec with a jewelers forceps (no: 5). One rat group was kept as control and the remaining were assigned as 1, 2 and 3 mT magnet . . .ic field treatment groups. The magnetic fields 1, 2 and 3 mT were applied to 30 rats for 4 hours per day for a week using Helmholtz coil systems. The control group was included in the system for 4 hours per day for a week while the system was turned off. Results: In terms of nerve regeneration, 3- and 7-days treatments of different magnetic field intensities had no statistically significant effect on amplitude and conduction velocity of correspound muscle action potential. Conclusions: The intensities and frequency of magnetic fields used in this study had no effect on conduction and excitability of nerves and consequently did not influence the nerve regeneration. Surgical anatomy and bifurcation patterns of the popliteal artery: An anatomical study

Objective: To clarify the anatomy of the popliteal artery and to determine key variations in the bifurcation patterns of the popliteal artery. Material and Methods: The popliteal fossae of 28 (12 right and 16 left) fixed lower extremities were carefully dissected, and the arterial pattern from the distal edge of the adductor hiatus (AH) to the proximal portion of each leg was documented. A digital caliper was used to measure the arterial length. The transverse plane between the distal edges of the femoral condyles (FCs), which is easily located, was used as reference. Results: The mean length of the popliteal artery (PA) from the AH . . . to the FCs was 9.26 ± 1.63 cm on the right side and 10.08 ± 2.12 cm on the left side, while the mean distance from the FCs to the site of bifurcation into the anterior and posterior tibial arteries was 7.20 ± 1.98 cm on the right side and 6.69 ± 1.15 cm on the left. The average arterial length from the level of the FCs to the site of origin of the peroneal artery was 10.01 ± 1.78 cm on the right side and 9.18 ± 1.07 cm on the left side, and the PA/extremity ratio was 0.21 ± 0.04 on the right side and 0.22 ± 0.04 on the left. Variable patterns of adult popliteal artery termination were observed. Conclusion: The data presented here can help reduce the complication rate and improve the success rate of both urgent and elective vascular procedures, which often require exposure of the popliteal artery. In this paper, we report a 43-year-old male patient presented with sudden decreased vision of both eyes. His visual acuity was 20/125 in the right and 20/400 in the left eye. A complete ophthalmologic examination was performed. Examination of fundus revealed multiple placoid, yellow-white lesions located deep in the retina of both eyes. The fluorescein angiogram showed early hypofluorescence and late hyperfluorescence of the lesions, typical of acute posterior multifocal placoid pigment epitheliopathy (APMPPE). Four days later, anterior chamber showed 2+ cells, mild flare, mutton-fat keratic precipitates on the corneal endothelium i . . .n both eyes and posterior synechia in the left eye. Radiographic examinations and laboratory tests were normal. Treatment with topical corticosteroids, cyclopentolate drop and oral prednisolone were administered. After six weeks, the patient's symptoms regressed and the visual acuities returned to 20/20. Treatment is not necessary for most cases of typical APMPPE; however, systemic and topical corticosteroids and/or cycloplegics may be useful for cases with atypical clinical features. © 2012 by Türkiye Klinikleri Daha fazlası Daha az

A case of hepatic lipoma: Case report [Karaciger lipomu olgusu]

Hepatic lipomas are extremely rare benign tumors. Because these lesions are asymptomatic, they are usually discovered incidentally. There is no risk of malignant degeneration. The imaging appearance of lipomas is characteristic. Computed tomography, magnetic resonance imaging, and sonographic findings of these lesions can help in characterization by allowing specific diagnosis. In computed tomography and magnetic resonance imaging, simple lipomas demonstrate fat attenuation or signal intensity and do not enhance after contrast material administration. A 46-year-old woman was admitted to the hospital because of dyspeptic symptoms. Li . . .ver ultrasound was suggestive of lipomas, whereas the diagnosis was confirmed by computed tomography and magnetic resonance imaging. Turkish version of the parenting stress index short form: A psychometric study

Objective: The aim of this study was to assess the psychometric details of the Turkish version of the Parenting Stress Index/Short Form (PSI/SF). Material and Methods: PSI/SF was administered to 90 mothers of children with congenital heart disease in the cardiology outpatient clinic. In 51 mothers the scale was re-administered 2 months later. Psychometric testing was carried out by using Cronbach's alpha, item-total correlations, test-retest reliability and validity. The concurrent validity of the instrument was established by correlating test scores with scores from 2 other well-known depression inventories (Beck and Zung Depressio . . .n inventories). Results: Psychometric analysis showed that the Turkish version of the PSI/SF demonstrated satisfactory internal consistency (Cronbach's alpha) for Total Stress Score (0.71) and also for subscales including Parental Distress (0.81), Parent-Child Dysfunctional Interaction (0.76) and Difficult Child (0.78). Test-retest correlations (r= 0.88) indicated good stability over a mean time period of two months. The PSI/SF was in good correlation with Beck Depression (r= 0.77) and Zung Depression (r= 0.69) inventories. Excessive dietary fructose is responsible for lipid peroxidation and steatosis in the rat liver tissues

Objective: Drinking large amounts of carbonated beverage is associated with increased risk for obesity and nonalcoholic steatohepatitis. Fructose is found in many fruits, vegetables, honey and in soft drinks, such as high fructose corn syrup. Excessive fructose consumption can adversely affect liver metabolism and increase lipid peroxidation, and this plays a role in the pathology associated with fructose feeding. Material and Methods: The present study reports the effects of high-fructose diet (10% and 20% in the drinking water) on the liver lipid peroxidation of rats. At the end of the experiment, biochemical evaluation of plasma . . .and histopathological evaluation of liver tissue were performed. Results: Enhanced thiobarbituric acid-reactant substance levels and abnormal lipid changes were observed in high-fructose-fed rats. In 10% fructose-fed group, the most consistent findings in the histologic sections of liver tissues were the hepatocellular degenerative changes, minimal macrovesicular and microvesicular steatosis in zone 1. In the 20% fructose-fed group, the trabecular liver structure was more seriously affected than the 10% fructose-fed group. High fructose-induced degenerative changes were evident in numerous hepatocytes of zone 1; the cells were enlarged and had a light and foamy cytoplasm filled with vacuoles. Conclusion: Our study suggests that fructose feeding is associated with lipid peroxidation and steatosis in liver and the dose of fructose is associated with the degree of tissue damage. Serum cystatin C levels in gastric cancer patients: Scientific letter

Elevated activities of cysteine proteinases in cancers are attributed to impaired regulation by the endogenous cysteine proteinase inhibitors (cystatins). Cystatin C is suggested to be a reliable marker of glomerular filtration rate (GFR). The purpose of this study was to evaluate whether cystatin C concentration was influenced by gastric malignancy, which is a common cancer type and the availability of this parameter safely for screening renal dysfunction in these patients. Since there is inadequate information on the clinical significance of cystatin C expression in human gastric cancers, we studied the differences in levels of se . . .rum cystatin C in this type of cancer and also evaluated them with respect to cancer stages. The levels of cystatin C in patients with gastric adenocancer were similar to those in the control group. Similarly, the values of serum creatinine and GFR in the patients were comparable to those of the controls. Serum creatinine values in controls and in patients did not show a statistically significant difference 0.8 ± 0.16 and 0.9 ± 0.35 respectively (p> 0.05). Serum cystatin C values in controls and patients were 0.74 ± 0.32 and 0.72 ± 0.6 respectively, which was also not different statistically (p> 0.05). When the patients were evaluated with respect to the stages of the cancer, the cystatin C values did not differ significantly. The mean ± SD levels of the patients in low and advanced stages were 0.79 ± 0.51 and 0.69 ± 0.64 respectively (p> 0.05). Our study clearly demonstrates that serum cystatin C has valuable potential for the detection and monitoring of GFR and may be safely used in gastric cancer patients. Varicella is a common and extremely contagious disease in childhood. The course of infection is usually mild to moderate in children but it may cause serious complications especially in immunosupressed adults and children. We presented a case with nosocomial varicella zoster virus infection in the form of chickenpox in a 67-year-old woman with rheumatoid arthritis (RA) for 15 years who was on treatment of methotrexate and prednisolon. Objective: If smoking habit among physicians in a country is common as it is in the general population, it is a particular problem for national tobacco control studies. The study aimed to investigate the smoking status of pulmonologists who are members of the Turkish Thoracic Society (TTS), and factors related with being a smoker. Material and Methods: Web-questionnaires were e-mailed to members of TTS by the TTS secretary from June 2010-February 2011. Only pulmonologists were included in the study. To analyse possible predictors of smoking smokers were categorized as ever smokers and never smokers. Possible predictors were sex, bei . . .ng graduated before/after 2004, urban/suburban working place, being academician or not, having education on smoking cessation help, experienced in smoking cessation outpatient clinic or not. Results: Web questionnaires of 699 pulmonologists were reviewed. Of the participants, 65.2% were female and the mean age was 39.4±9.0 (25-72); 69% were non-smokers, 21.1% were ex-smokers, and 9.9% were current smokers. Never smokers were mostly included in the group of pulmonologists younger than 40 years old (p=0.001). The number of never smokers with experience in smoking cessation outpatient clinic was higher than the number without any experience (72.1% and 67.1%, respectively; p=0.03). Smoking was 1.8-fold more likely among males (95% confidence interval-CI 1.2-2.5) and 1.8-fold more likely among those graduated before 2004 (95% CI 1.1-3.1); 66.7% of current smokers were in the contemplation stage. The number of cases in contemplation stage were mostly among regular smokers (p=0.001). Conclusion: The results of this study suggesting relatively low prevalence of smoking among pulmonologists are important for tobacco control. © 2013 by Türkiye Klinikleri Daha fazlası Daha az

A case of Klatskin tumor with high levels of CA 19-9

Carbohydrate antigen 19-9 (CA 19-9) is used as a serum tumor marker for adenocarcinoma of the upper gastrointestinal tract, particularly primary adenocarcinoma of the pancreas. In addition, this tumor marker can be highly elevated in benign diseases such as acute pancreatitis, chronic pancreatitis, chronic liver disease and biliary tract disease. A 69 year-old man was hospitalized with jaundice, colacolored urine, right-sided upper abdominal pain, weight loss and pruritus. Laboratory data revealed a high level of serum CA19-9 of more than 100 000 U/mL and a mass in the left lobe of the liver was detected on computed tomography. ERCP . . ., PTCA and CT-guided biopsy yielded the diagnosis of Klatskin tumor. Review of the literature revealed that Klatskin tumor with high levels of CA 19-9 over 100 000 U/mL was not reported. Sarcoidosis is a multisystem disorder of unknown etiology, which is histopathologically characterized by noncaseating granulomas. Cutaneous sarcoidosis localizing on the auricle may be observed in various clinical types. Angiolupoid sarcoidosis is a rare form of cutaneous sarcoidosis.
