The aetiology in paediatric aphakic glaucoma

Purpose To investigate the causes of glaucoma in children following removal of cataracts. Methods In total, 24 patients (37 eyes) with uncomplicated congenital cataracts who developed glaucoma following cataract removal were studied retrospectively. Cataract morphology, surgical technique, postoperative complications, time to glaucoma onset, gonioscopic findings, the presence of microcornea, and the histopathologic characteristics of the filtration angle in one case were the studied parameters. Results We found a bimodal onset of glaucoma. Early-onset glaucoma occurred at a mean age of 6 months in 15 eyes and delayed-onset glaucoma . . .at a mean age of 12 years in 22 eyes. Early-onset glaucoma was significantly (P = 0.018) more likely to be due to angle closure. With delayed-onset glaucoma, the filtration angle is open in 86% of eyes and significantly (P = 0.006) more eyes in the delayed-onset group had microcornea. Conclusions Performing cataract surgery very early in life in microphthalmic eyes and leaving residual lens material increases the risk for glaucoma. We recommend a prophylactic iridectomy in eyes at risk for pupillary block. Eyes with delayed-onset glaucoma have open filtration angles but with findings consistent with incomplete development of filtration structures. Background and objective: This trial was designed to investigate the effects of the interleukin (IL)-1 beta, IL-1Ra, IL-2, IL-6, IL-10 gene polymorphisms on Behcet's disease (BD) occurrence and the association between the polymorphisms and the phenotype. Materials and method: The study population consisted of 71 patients and 70 age and gender-matched healthy subjects. Each of the participants had 2cc of blood withdrawn, which was placed into a whole blood tube, and the DNA was obtained using the NucleoSpin (R) Blood DNA Isolation kit. Results: There were no significant differences between the groups with respect to the IL-1Ra, IL-1 beta, IL-2, IL-6 and the IL-10 gene polymorphism distributions. In the patient group the IL-1RN2 gene polymorphism was detected to be statistically correlated with the presence of articular involvement (p = 0.0283) and the IL-1 beta gene polymorphism was statistically correlated with the presence of an ocular lesion (p = 0.0178). The evaluation of the IL-2 gene polymorphism (p = 0.0065) and IL-10 gene polymorphism (p = 0.0483) distributions with respect to age of BD onset revealed a statistically significant distribution. Conclusion: The statistical correlations between the articular involvement and IL-1RN, the ocular involvement and the IL-1 beta, and the age of disease onset and the IL-2 and IL-10 gene polymorphisms, detected for the first time in the literature, suggest that these polymorphisms could be statistically associated with the disease symptoms and used as prognostic factors. Psammoma bodies are round, concentrically laminated calcospherites ranging from 5 to 100 pm in diameter. They may indicate certain types of tumors including papillary carcinoma of the thyroid, meningioma and papillary serous tumors of the ovary, and to a lesser extent may be found in leiomyomas and angiomas of the gastrointestinal tract. Dystrophic calcification is uncommon in gastric cancer and displays either diffuse or psammomatous pattern. Diffuse type calcification is generally seen within the pools of mucin in advanced mucinous adenocarcinoma. Gastro esophageal reflux disease (GERD) is the most common gastrointestinal disorder and often is associated with hiatal hernia (HH). Nissen fundoplication is the most common surgical treatment method. Despite surgical treatment, recurrence rate is still high. In this study, we aimed to identify the importance of the mesh shape in preventing recurrence after Nissen fundoplication. A hundred twenty two patients who operated Nissen fundoplication owing to GERD and/or HH were evaluated. Nissen fundoplication was made all patients. Patients were divided into three groups according to hiatoplasty procedure; group 1 (V-shaped mesh), group . . . 2 (V-shaped mesh + Fibrin glue), and group 3 (special designed mesh, Kar's mesh). Groups were compared regarding intraoperative, postoperative early-and long-term complications. Mean age was 42.75 years, and male to female ratio was 1: 2.98. The mean follow-up period was 27 mounts. There was no mortality during follow-up. The most common presenting symptom was heartburn (93.4%). There wasn't difference between groups in terms of the intraoperative complications and postoperative early-term complications. The overall recurrences rate was 4.9% and dysphagia > 3 months rate was 1.6%. No recurrence was not observed in group 3, while recurrence was observed in 4 patients in group 1 (P = 0.030). Patients should be carefully selected for surgery because complication rate is high despite successful anti-reflux surgical treatment. In this study, we have used a special designed mesh. In the treatment of developmental dysplasia of the hip, concentric reduction of the femoral head into the acetabular cavity plays a key role in the natural development of the acetabulum. Objectives: Atrial fibrillation (AF) is one of the most common causes of preventable ischemic stroke and is related to increased cardiovascular morbidity and mortality. There is a lack of data in Turkey on the use of new oral anticoagulants (NOACs), and time in therapeutic INR range (TTR) in vitamin K antagonist users and AF management modality. In this multi-center trial, we aimed to analyze, follow and evaluate the epidemiological data in non-valvular AF patients. Study design: Four thousand one hundred consecutive adult patients from 42 centers with at least one AF attack identified on electrocardiography will be included in the . . .study. Patients with rheumatic mitral valve stenosis and prosthetic valve disease will be excluded from the study. Influence of soil compaction on microfungal community structure in two soil types in Bartin Province, Turkey

Soil compaction negatively influences physical properties of soil (bulk density and pore space), and may consequently limit soil microfungi, which are significant for nutrient bioavailability. We measured microfungal community responses to compaction in a sandy loam and a clay loam soil at picnic sites. Soil bulk density increased significantly in the compacted samples. However, microfungal numbers and community composition were unrelated to changes in soil bulk density. With increases in bulk density from 1.22 to 1.37 g cm(-3) in the clay soil and from 1.38 to 1.54 g cm(-3) in the sandy loam soil, the total number of fungi declined . . . or showed insignificant increases. In the compacted samples as well as the control sites, the most frequently occurring genera in the clay soil were Penicillium, Aspergillus, and Gliocladium. However, the most prominent feature occurring within the sandy loam soil was the exhibition of the greatest increase in the frequency of the Fusarium genus. When comparing compacted and control soils, fungal community composition corresponded more closely within each soil texture. The two microfungal soil communities, therefore, tolerated compaction. In contrast, a difference occurred in the fungal communities between the two soil textures. Background: Ventricular premature beats (VPBs) are one of the most common rhythm abnormalities. Structural heart diseases such as myocardial hypertrophy and left ventricular dysfunction are associated with VPBs. However, the exact mechanism of VPBs in patients without structural heart disease has not been revealed yet. Epicardial fat tissue (EFT) is a visceral fat around the heart. Increased EFT thickness is associated with myocardial structural and ultrastructural myocardial abnormalities, which may play a role in the development of VPBs. Aims: To evaluate the possible relationship between EFT thickness and frequent VPBs. Methods a . . .nd results: The study population consisted of 50 patients with VPBs and 50 control subjects. Frequent VPBs were defined as the presence of more than 10 beats per hour assessed by 24-h Holter electrocardiography monitoring. EFT thickness was measured by transthoracic echocardiography. Multivariable logistic regression analysis was used to assess factors related with frequent VPBs. Baseline demographic and biochemical features including age, gender, and rates of hypertension and diabetes mellitus were similar in both groups. EFT thickness was significantly higher in patients with frequent VPBs than in controls (3.3 +/- 1.3 mm vs. 2.2 +/- 0.8 mm, p < 0.001). In multivariable logistic regression analysis, EFT thickness was independently associated with VPB frequency (B = 1.030, OR = 2.802, p < 0.001).
