Objectives: Inflammation plays an important role in the pathogenesis of metabolic syndrome (MS). We investigated the effect of fluvastatin treatment on inflammatory markers in patients with MS. Study design: The study included 47 patients (36 females; 11 males; mean age 55±8 years) with MS. The diagnosis of MS was based on the presence of at least three criteria of the NCEP ATP III guidelines. All the patients received 80 mg fluvastatin treatment for six weeks. Laboratory parameters were measured before and after treatment, and flow cytomet-ric analysis of peripheral blood leukocytes was performed. The results were compared with tho . . .se of 47 age- and sex-matched healthy controls (33 females, 14 males; mean age 52±8 years). Results: Fluvastatin treatment resulted in significant decreases in levels of total cholesterol, LDL cholesterol, triglycer-ide ( Daha fazlası Daha az

Objectives: To determine whether angiotensin and endothelin have any role in testicular ischemia reperfusion injury by investigating the effects of the angiotensin converting enzyme inhibitor enalapril, selective non-peptide angiotensin-II type I blocker losartan and dual endothelin receptor blocker bosentan. Methods: Rats were anesthetized with thiopental sodium (50mg/kg i.p.) before the operation. The left testicular artery and vein of rats were occluded for 1h; before the bilateral orchiectomy, the organ was allowed to reperfuse for 3h or 24h. Enalapril (20mg/kg i.p.), losartan (30mg/kg i.p.), bosentan (10mg/kg i.p.) or vehicle ( . . .saline) were given 30min before reperfusion. Malondialdehyde level was measured in testicular tissue after 3h of reperfusion. Histological examination was carried out after 24h of reperfusion. Results: Ischemia reperfusion caused a significant increase in malondialdehyde level of ipsilateral testis, and histopathological injury in both ipsilateral and contralateral testes. Enalapril, losartan and bosentan treatments prevented the ischemia reperfusion-induced augmentation in malondialdehyde levels. Only bosentan treatment ameloriated ischemia reperfusion-induced histopathological alterations. Conclusions: Endothelin might play a more important role in pathogenesis of testicular ischemia reperfusion injury when compared with angiotensin. © 2011 The Japanese Urological Association Daha fazlası Daha az

Article | 2013 | Herz38 ( 3 ) , pp.299 - 305

OBJECTIVES: The aim of this study was to compare the effects of the new generation ß-blocker anti-hypertensive drugs carvedilol and nebivolol on aortic elastic properties which are important indicators of hypertension-related morbidity and mortality.METHODS: A total of 50 patients who had been diagnosed with stage 1 hypertension according to the Joint National Committee (JNC) VII criteria and who had not received any anti-hypertensive treatment were enrolled in this study. Patients were randomized to receive either 25 mg/day carvedilol (n=25) or 5 mg/day nebivolol (n=25) for 3 months at the beginning of the study. Three patients (1 . . .in the carvedilol group, 2 in the nebivolol group) who did not attend 3 month follow-up measurements were excluded from the study. The study was completed with 47 patients (25 women; mean age: 49 ± 9 years). The aortic elastic parameters such as aortic strain (AS), aortic distensibility (AD), and aortic stiffness index (ASI) were measured by echocardiography.RESULTS: Carvedilol and nebivolol provided a similar decline in both systolic and diastolic blood pressures (-12/-7 mmHg, Daha fazlası Daha az

Article | 2014 | Turkish Nephrology, Dialysis and Transplantation Journal23 ( 1 ) , pp.20 - 25

Objective: Cardiorenal syndrome (CRS) describes a dysregulation of the heart and kidneys affecting each other. Recently hemodialysis treatments were used more frequently. Aim was to analyze the effects of conventional diuretic and UF treatments. Material and Methods: Thirty-four Type 4 CRS diagnosed patients were included. Baseline characteristics were recorded. Echocardiography measured at the admission and at the end of the treatment. Results: The mean age 67.4±9.3 (51-93) years and follow-up period were 15.9±11.5 months. The patients were grouped as diuretic group, n=12 and UF group, n=22. At the beginning mitral valve A wave, bl . . .ood urea nitrogen and creatinine values were higher in the UF group while creatinine values were higher in the UF group compared to diuretic group at the end of the study. Although basal ejection fraction (EF) values were not different, it was higher in the UF group at the end of the study (42.38±12.70 % and 29±3.67 %, p 0.05). Conclusion: In Type 4 CRS, mortality and hospital admissions were not reduced by UF treatment but cardiac function assessed by EF was significantly improved suggesting this therapy to be beneficial in appropriate patients Daha fazlası Daha az

An asymptomatic needle in the left ventricular anterolateral wall: A prison inmate's strange radio antenna

Akpinar I. | Sayin M.R. | Karabag T. | Dogan S.M. | Aydin M.

Article | 2012 | Echocardiography29 ( 8 ) , pp.20 - 25

A foreign body such as a needle in the heart can be life-threatening. While this may occur accidentally, needles may be inserted into the body by psychiatric patients or in cases involving domestic violence. A needle can migrate through the thorax toward the heart. In drug users, needles may also reach the right ventricle via the peripheral veins. Cardiac injury can occur via the esophagus after swallowing a needle. The clinical outcome may vary from an asymptomatic situation to tamponade or shock, depending on how severely the cardiac structures are affected. In injuries involving the thorax, pneumothorax may cause sudden shortness . . . of breath. Here, we report the case of a 34-year-old male prison inmate who accidentally lodged a pin in his left ventricle while asleep. As he has refused surgery, it was decided to follow the patient carefully. © 2012, Wiley Periodicals, Inc Daha fazlası Daha az
