Article | 2016 | International Journal of Endocrinology2016

Objectives. The aim of our study was to investigate the differences in adrenal gland volume between nondiabetic controls and Type-2 diabetic patients and to examine the influence of glycemic control in diabetes mellitus on adrenal gland volume. Methods. From March 2 to November 25, 2015, 62 consecutive patients with Type-2 DM along with 62 nondiabetics matched by age, gender, and BMI were enrolled in this prospective study. Our diabetes patients were categorized into two groups, well-controlled and poorly controlled diabetes groups. Adrenal volumetric measurements were performed by two radiologists, prospectively and independently, . . .with semiautomatic software. Interobserver reliability was studied using the interobserver correlation coefficient (ICC). Results. The total adrenal volume (TAV) was significantly higher in Type-2 diabetic patients when compared with nondiabetic patients (p < 0.05). When we investigated diabetic patients according to glycemic controls, the TAVs in controlled diabetic patients were significantly higher than in those of the poorly controlled or uncontrolled diabetic patients (p < 0.05). Nondiabetic control patients have significantly smaller TAVs when compared to controlled and poorly or noncontrolled diabetic patients (p < 0.05). Conclusion. Our study suggests that adrenal gland volume measurement may be used as an indirect marker of glycemic control in patients with diabetes. © 2016 Ismail Serifoglu et al Daha fazlası Daha az

Acute kidney injury related to intravenous immunoglobulin therapy: A case report [Intravenöz immünglobulin tedavisi ile ilişkili akut böbrek hasari: Olgu sunumu]

Article | 2017 | Turkish Nephrology, Dialysis and Transplantation Journal26 ( 2 ) , pp.232 - 235

Intravenous immunoglobulin (IVIG) has been developed for the treatment of hypogammaglobulinemia and is also used for many systemic inflammatory and autoimmune disorders. Acute kidney injury is a rare complication of the use of IVIG, and is mainly caused by stabilizing agents in the protein formulation. A complete recovery of renal function is seen within days. We present the case of an 81-year old woman, treated for acute inflammatory demyelinating polyneuropathy (Guillain-Barré syndrome) that caused loss of sensation and muscle weakness in the leg. She was treated with IVIG for five days. Renal failure developed during treatment. T . . .he patient's acute kidney injury resolved a month later. The patients should be monitored closely after IVIG therapy for renal injury Daha fazlası Daha az

Red Cell Distribution Width Has a Predictable Value for Differentiation of Provoked and Unprovoked Venous Thromboembolism

Article | 2016 | Indian Journal of Hematology and Blood Transfusion32 ( 4 ) , pp.481 - 487

Venous thromboembolism (VTE) is generally classified as provoked or unprovoked. This dichotomy is important for following patients, mortality rate, prognosis and whether more efficient therapy is needed. In VTE patients, during initial diagnosis, it is not known exactly whether red cell distribution width (RDW) have a predictable value for this differentiation and pathogenesis. In this study, 298 patients with VTE and 197 control subjects were included. Patients with VTE were defined as provoked or unprovoked with respect to physical examination findings and laboratory values. Changes in RDW were tested between VTE patients and cont . . .rol subjects, provoked and unprovoked VTE patients, and separately with control subjects. RDW was found to be high in provoked and unprovoked groups compared with control group (p Daha fazlası Daha az

Purpose: Hepatocellar carcinoma (HCC) remains a major health problem being the third leading cause of deaths due to cancer worldwide. Because HCC is known to be highly resistant to conventional systemic therapies, single-agent or combination of systemic therapies have been investigated Today, sorafenib, a multikinase inhibitor, is the only approved systemic agent for the first line treatment of advanced HCC. In this study, we aimed to investigate the influence of different concentrations of cisplatin, doxorubicin, pegylated doxorubicin (PLD), oxaliplatin and gemcitabine by applying these agents either single or in combinations on ma . . .hlavu cell line. Methods: HCC mahlavu cell line was used for the experiments. Cell death was measured by flow cytometry at 48 hrs after incubation with various concentrations (0.1 uglml, 1.0 uglml and 10 uglml) of the drugs. Results: Cell death due to gemcitabine was found to be significantly higher than cell deaths caused by the other single agents including cisplatin, oxaliplatin, doxorubicin and PLD ( Daha fazlası Daha az

Objectives: In this study, we aimed to evaluate the one-year follow-up findings of endovascular treatment with drug-covered balloon in patients with femoropopliteal artery disease (FAD). Patients and Methods: From June 2012 to March 2015, 43 patients were enrolled in the study. Fifteen patients, 16 limbs and 19 lesions were treated with percutaneous transluminal angioplasty (PTA). Twenty-eight patients, 31 limbs and 36 lesions were treated with drug-coated balloon (DCB). At 12 months, all patients were examined with magnetic resonance angiography. Results: Primary patency was seen in 47.4% of the treated lesions in the PTA group and . . . 80.5% in the DCB group. There was statistically significant difference in primary patency between the groups (P = 0.011). Clinical improvement was seen in 56.3% of the treated limbs in the PTA group and 83.9% in the DCB group. There was no statistically significant difference in clinical improvement between the two groups (P = 0.075). Conclusion: DCB with low restenosis rate can be used safely for endovascular treatment in patients with FAD as an alternative of standard balloon and stent. © 2017, Iranian Journal of Radiology Daha fazlası Daha az

Oz I.I. | Bilici M. | Serifoglu I. | Arpaci D.K. | Buyukuysal M.C. | Bayraktaroglu T.

Article | 2017 | Acta Endocrinologica13 ( 2 ) , pp.168 - 173

Purpose. We aimed to assess the relationship between the regional body fat distribution and insulin resistance and pancreas volume (PV) in type-2 diabetes (DM) patients. Methods. Fifty-three consecutive type-2 diabetic and 51 non-diabetic patients matched by age, gender and body mass index (BMI) were enrolled. Subcutaneous adipose tissue (SAT), visceral adipose tissue (VAT), waist circumference, and PV were measured with computed tomography. Insulin resistance was assessed by the homeostasis model assessment of insulin resistance (HOMA-IR). Results. Patients with type-2 DM had significantly lower PV than non-diabetic individuals. HO . . .MA-IR ranged from 0.74 to 6.24; and from 0.37 to 3.26, in type-2 DM patients and non-diabetics, respectively. VAT was positively correlated with HOMA-IR in two groups. There were inverse correlations between PV and VAT and VAT/SAT but only in diabetics. Conclusions. The VAT/SAT ratio may reflect the possible role of VAT to better understand the pathogenesis of obesity-related disorders in patients with type-2 DM. © 2017, Acta Endocrinologica Foundation. Oz I.I. | Yucel M. | Bilici M. | Şerifo?lu I. | Sayin R. | Ilikhan S.U. | Acikgoz M.

Article | 2016 | Journal of Stroke and Cerebrovascular Diseases25 ( 2 ) , pp.404 - 409

Background The objective of the study is to evaluate the reliability of mean platelet volume (MPV) for predicting ischemic stroke (cerebrovascular event [CVE]) among patients with different degrees of carotid stenosis. Methods Fifty-two patients with CVEs, 136 patients with carotid artery disease (CAD), and 40 healthy volunteers were enrolled in this study. All participants were divided into the following groups according to CAD: absence of stenosis, less than 50% stenosis, 50%-69% stenosis, and 70% or more to total occlusion of the internal carotid artery. For each participant, the parameters of CAD were assessed using ultrasonogra . . .phy. To obtain the values of CRP and MPV and the leukocyte and platelet counts, all samples were processed within 30 minutes after blood collection. Univariate and multivariate analyses were used to evaluate the association between the values of serum C-reactive protein (CRP) and MPV and carotid stenosis. Results In terms of age and gender, there was no statistically significant difference between the groups (P =.094 and P =.428, respectively). However, CRP values in patients with CAD and CVEs were significantly higher than those in the controls (P Daha fazlası Daha az

Article | 2015 | Respiratory Medicine Case Reports14 , pp.1 - 3

Tuberculosis (TB) has become a global emergency worldwide. The long time period between the exposure to TB bacillus and the onset of symptoms cause a delay in diagnosis. Herein, we report a case of 64-year-old female patient suffering from dyspepsia, anorexia, weight loss and abdominal pain for the last 8 months. Physical examination, ascites fluid evaluation, chest radiography, ultrasonographic and tomographic scans, histopathological analysis of the lymphadenopathy (LAP) and endometrial tissue revealed TB. A fourfold antituberculous treatment with isoniazid, pyrazinamide, rifampicin and ethambutol was prescribed for two months and . . . for four months maintenance therapy with isoniazid and rifampicin was given. On the fourth month of the medical treatment the patient clinically recovered. Since the diagnosis of TB is difficult, high grade suspicion, combination of the radiologic, microbiologic and histopathological examinations are needed to achieve a diagnosis. © 2014 The Authors Daha fazlası Daha az

Article | 2019 | Journal of Medical Ultrasonics46 ( 1 ) , pp.45 - 49

Purpose: To evaluate the sonoelastographic changes in the Achilles tendon in patients with chronic renal failure (CRF) using virtual touch imaging quantification (VTIQ) elastography. Methods: Twenty-six patients undergoing three hemodialysis sessions per week and 26 subjects admitted to our institution between January 2016 and April 2016 were included in this prospective study. The characteristics and body mass index of the patients were noted. Ultrasonography was performed parallel to the long axis of the bilateral Achilles tendons during relaxation of the legs using the Siemens Acuson S3000™ ultrasound device (Siemens HealthCare, . . .Erlangen, Germany). Tendon thickness was reviewed, and tissue stiffness was quantitatively assessed using VTIQ elastography. Independent samples t test and Mann–Whitney U test were used for statistical analyses. Results: The median values of shear wave velocities of the Achilles tendon in patients with CRF were 7.19 m/s (4.23–9.77 m/s) on the right and 6.98 m/s (4.00–9.82 m/s) on the left, while they were 5.11 m/s (4.09–8.82 m/s) on the right and 5.36 m/s (4.05–8.80 m/s) on the left in controls. The stiffness of the Achilles tendons in patients with CRF was found to be higher than that in controls (right: P < 0.001, left: P = 0.004). There was no statistically significant difference in tendon thickness between the CRF and control groups (P > 0.05). Conclusion: The thickness and stiffness of tendon can be effectively evaluated with sonoelastography. The thickness of the Achilles tendon did not significantly differ between the patients with CRF and healthy subjects. However, the stiffness of the Achilles tendon measured with VTIQ elastography was demonstrated to be increased in the patients with CRF. © 2018, The Japan Society of Ultrasonics in Medicine Daha fazlası Daha az

Article | 2015 | Turkish Journal of Gastroenterology26 ( 6 ) , pp.456 - 460

Background/Aims: In our study, we aimed to assess the effect of vitamin E and C supplementation to triple and quadruple Helicobacter pylori eradication regimens. Materials and Methods: Four hundred patients with H. pylori infection were classified into four groups. Patients in group A (n=100) received amoxicillin, clarithromycin, and lansoprazole for 2 weeks. In group B, patients (n=100) received vitamins C and E for a month, in addition to amoxicillin, clarithromycin, and lansoprazole for 2 weeks. Patients in group C (n=100) received amoxicillin, clarithromycin, lansoprazole, and bismuth subcitrate for 2 weeks, whereas those in gro . . .up D (n=100) received vitamins C and E for a month, in addition to amoxicillin, clarithromycin, lansoprazole, and bismuth subcitrate for 2 weeks. H. pylori eradication was assessed with the C14 urea breath test 2 months after the end of the therapy. The eradication rate was assessed using per-protocol (PP) and intention-to-treat (ITT) analyses. Results: Three hundred forty-eight patients finished the study. The eradication of H. pylori was achieved in 63 of 84 patients (75%) by PP and 63 of 100 (63%) by ITT analysis in group A, 60 of 84 (71.4%) by PP and 60 of 100 (60%) by ITT analysis in group B, 72 of 89 (80.9 %) by PP and 72 of 100 (72%) by ITT analysis in group C, and 76 of 91 (83.5%) by PP and 76 of 100 (76%) by ITT analysis in group D. There was no remarkable change between groups A and B (p>0.05). Similar results were also found between groups D and C (p>0.05). Conclusion: This study revealed that supplementing vitamins C and E to either the triple or quadruple therapies did not provide an additional advantage for achieving significantly higher eradication rates for H. pylori. © Copyright 2015 by The Turkish Society of Gastroenterology Daha fazlası Daha az

Article | 2017 | Iranian Red Crescent Medical Journal19 ( 6 ) , pp.456 - 460

Background: A decrease of naive T-cells and a concomitant increase in memory cells are the accepted consequences of aging on the adaptive immune system. Objectives: The current case-control study aimed at considering the impact of chronic hyperglycemia that leads to glycation of modified self proteins on aging via the memory cell percentages among the patients admitted to the endocrinology department of Bulent Ecevit University Hospital in Zonguldak province, Turkey, from September to October 2015. Methods: Blood samples were collected from 81 patients with diabetes mellitus (DM) and 39 healthy volunteers with no history of autoimmu . . .ne diseases or chronic inflammatory disorders, based on the purposive sampling method. The patients were divided into 2 groups based on the presence and absence of the diabetic microvascular complications. Diabetic nephropathy, neuropathy, and retinopathy were investigated according to the American Diabetes Association criteria. T-lymphocytes subpopulations were measured in peripheral blood by the flow cytometry. CD27, CD45 RO, and CD45 RA were used to discriminate naive and memory T-cells (CD4+ and CD8+). Data for each subpopulation were reported as a percentage of the total CD4+ and CD8+ cells. Results: The mean percentage of naive (CD45RA + CD27+) and memory (CD45RO + CD27+ central memory and CD45RO + CD27-effector memory cells) CD4+ T-cells in patients with type 2 DM and healthy controls were 26.73 ± 15.04, 19.21 ± 10.80, 31.35 ± 10.94, and 15.07 ± 6.97, respectively. A decrease in the naive and an increase in memory CD4+ T-cell proportion were found in patients with type 2 DM, compared to the healthy controls (P values = 0.031 and 0.018, respectively). Conclusions: Higher memory and lower naive CD4+ T-cells probably reflect thr accelerated aging of the adaptive immune system in patients with type 2 diabetes. © 2017, Iranian Red Crescent Medical Journal Daha fazlası Daha az

Does the awareness of the patient about the amount of daily salt consumption decrease his/her salt intake? [Hastanın günlük ne kadar tuz tükettiğini bilmesi tuz tüketimini azaltır mı?]

Bilici M. | Yılmaz F. | İlikhan S.U. | Borazan A.

Article | 2016 | Medeniyet medical journal31 ( 4 ) , pp.237 - 240

In the present study, we investigated the impact of the awareness of the daily salt intake on dietary salt consumption in patients with primary hypertension. One hundred and fifty-four patients (male, 80, and female, 74) treated with the established diagnosis of hypertension, and also followed up after providing information about salt consumption were enrolled in the present study. Urinary sodium was measured in 24-hour urine samples, and the patients were informed about their salt consumption together with their laboratory results. Three months later urinary sodium levels were measured in 24-hour urine samples again. Changes in blo . . .od pressure, and salt consumption were compared statistically. Mean age of 154 patients enralled in the study was 48.4±14.9 years. It was observed that the patients had not known their daily salt consumption. Their daily salt intake had been 12.6±4.8 g, while their daily intake dropped to 7.4±3.7 g after control of salt intake with monitorization of urinary sodium. Baseline systolic/diastolic blood pressure (p=0.024 and =0.034, respectively) values decreased, in parallel with reduction in daily salt intake (12.6±4.8 g/day to 7.4±3.7 g/day) ( Daha fazlası Daha az
