Aim: Leptin is an important controller of the size of fat stores by inhibiting appetite. In wrestling, fat metabolism is important not only for aerobic energy system, but also for weight control and weight loss before competition. The aim of this study was to measure and compare the serum leptin levels, glucose homeostasis, and serum lipoproteins between Turkish national wrestlers and sedentary males. Materials and methods: Forty-five Turkish national wrestlers at the end of Olympic training camp before Beijing 2008 Olympic Games and 43 sedentary students were selected as subjects. Post-exercise levels of serum leptin, HOMA (Homeost . . .asis model assessment) values, triglycerides, total cholesterol, LDL-cholesterol, HDL-cholesterol, and VLDL-cholesterol were measured after overnight fasting. A Mann-Whitney U test was used to test the differences between 2 groups. Pearson Correlation was conducted between leptin and the other parameters. Results: No significant difference was observed among the means of leptin and total cholesterol levels of the 2 groups. Wrestlers had significantly higher fasting insulin, HDL-cholesterol, triglyceride, and HOMA levels, but a lower LDL-cholesterol level compared to the sedentary males. The positive correlation was only found between leptin and fasting insulin levels (r = 0.310, P = 0.043). Conclusion: This study indicated that wrestling training had no effect on resting leptin level after overnight fasting and wrestlers had also decreased insulin sensitivity with higher fasting insulin, HDL-cholesterol, triglyceride, and HOMA levels compared to sedentary males. © TÜBİTAK Daha fazlası Daha az

The association between insulin resistance and hepatic fibrosis in patients with chronic hepatitis C: An observational, multicenter study in Turkey

Background/Aims: To evaluate the association between insulin resistance and hepatic fibrosis in patients with chronic hepatitis C.Materials and Methods: A total of 104 chronic hepatitis C patients were included in this non-interventional, open-label, observational, multicenter, cross-sectional study conducted at 20 gastroenterology clinics in Turkey. The primary end point was the correlation between stage of hepatic fibrosis and insulin resistance evaluated via the homeostasis model of assessment-insulin resistance index. Confounders of hepatic fibrosis and insulin resistance were the secondary end points.Results: The mean age of pa . . .tients was 52.8 years; 65.4% were female. Type 2 diabetes was present in 6.8% and insulin resistance noted in 38.0% of patients. Further, 45.7% of the patients had mild (A0/A1) and the remaining had moderate/severe (A2/A3) hepatic necroinflammatory activity. Patient distribution according to Metavir fibrosis stage was as follows: F0/F1 (57.0%); F2 (6.5%); F3 (23.7%); and F4 (12.9%). A univariate analysis revealed significant positive correlations between Metavir fibrosis stage and insulin resistance (r=0.297; p=0.007). Logistic regression analysis showed that significant predictors of insulin resistance were high alanine transaminase levels (odds ratio, 0.97; 95% confidence interval, 0.944-0.997) and liver fibrosis stage (odds ratio, 0.114; 95% confidence interval, 0.021-0.607).Conclusion: Our findings revealed significant associations between insulin resistance and hepatic fibrosis. © 2014 by The Turkish Society of Gastroenterology Daha fazlası Daha az

Aim: The aim of this study was to assess the insulin resistance and serum leptin levels in patients with obstructive sleep apnea syndrome (OSAS), and to compare body mass indexes (BMI) of OSAS patients with matched controls without OSAS. Method: Twenty eight patients having apnea-hypopnea index (AHI)?5 included in the study. Thirty two healthy subjects assumed as a control group. Venous blood was obtained in the fasting state for the measurement of glucose, insulin and leptin levels. Insulin resistance index was based on the homeostasis model assesment method (HOMA-IR). Result: There was no significant difference in the serum leptin . . . levels (control group, 32.88±24.22 ng/ml, OSAS group, 24.93±25.84 ng/ml) and HOMA-IR (control group, 3.01±1.81, OSAS group, 2.58±1.21) between control group and OSAS patients. Insulin resistance and circulating plasma leptin concentrations in OSAS patients were independent of the AHI and were not different from the control group. Conclusion: We concluded that insulin resistance and plasma leptin concentrations are mostly associated with the degree of obesity and BMI. Helicobacter pylori causes a lifelong infection in the stomach after exposure. H. pylorihas been shown to be associated with peptic ulcer and gastric cancer development. Moreover, it is held responsible for some other nongastric diseases. Among them, coronary heart disease attracts much debate. Many studies have demonstrated a close relationship between insulin resistance and atherosclerosis. Chronic inflammation and alterations in counter-regulatory hormones are deemed responsible for the etiology of insulin resistance. We aimed to examine the effect of H. pylori on insulin resistance. HOMA-IR (homeostasis model assessment of insulin resistance) level was used to assess insülin resistance. Thirty-six patients were H. pylori positive and 27 were H. pylori negative. There was no difference between the two groups with regard to age, gender, or body mass index. HOMA-IR level was 1.73± 1.1 in the H. pylori-negative group, whereas it was 2.56 ± 1.54 in the H. pylori-positive group (P < 0.05). Purpose. We aimed to assess the relationship between the regional body fat distribution and insulin resistance and pancreas volume (PV) in type-2 diabetes (DM) patients. Methods. Fifty-three consecutive type-2 diabetic and 51 non-diabetic patients matched by age, gender and body mass index (BMI) were enrolled. Subcutaneous adipose tissue (SAT), visceral adipose tissue (VAT), waist circumference, and PV were measured with computed tomography. Insulin resistance was assessed by the homeostasis model assessment of insulin resistance (HOMA-IR). Results. Patients with type-2 DM had significantly lower PV than non-diabetic individuals. HO . . .MA-IR ranged from 0.74 to 6.24; and from 0.37 to 3.26, in type-2 DM patients and non-diabetics, respectively. VAT was positively correlated with HOMA-IR in two groups. There were inverse correlations between PV and VAT and VAT/SAT but only in diabetics. Conclusions.
