Dissecting cellulitis in a white male: Response to isotretinoin

A 22-year-old white male with a one-year history of dissecting cellulitis of the scalp was successfully treated with oral isotretinoin. Complete healing was seen after 2 months and the treatment was discontinued after a course of 9 months. The patient is still in remission 3 months after cessation of the therapy.

Flexor tendon injuries of children in zone 2 (FDS repair or not).

Twenty flexor tendons in 13 children with a mean age of 7 years old (range from 9 months to 14 years) were treated by primary suture between 1994-1999. Both tendons were repaired in 12 (60%) digits and only FDP in 8 (40%) with excision of FDS. Mean follow-up was 19 months (from 6 to 55 months). In the 12 fingers with FDS repair, the results were excellent in 2, good in 4, fair in 3 and poor in 3. In the 8 fingers with FDS excision, 2 rated excellent, 5 good and 1 poor. In conclusion, repair of FDP alone with regional excision of FDS simplifies the operative procedure and reduces the number of tendon sutures inside the sheath.

Purpose: To investigate whether restoration of normal stereo acuity is possible in refractive accommodative esotropia. Methods: Twenty patients with refractive accommodative esotropia were included in the study. The cycloplegic refraction and the best corrected visual acuity of each patient were recorded. The patients were evaluated by the 4-prism-diopter test, Bagolini striated glasses, the Worth 4-dot test, the TNO stereo test, and the stereo fly plate of the Titmus test. Relationship between aims and respiratory muscle strength endurance in patients with rheumatoid arthritis

Patients with rheumatoid arthritis have reduced skeletal muscle strength measured with handgrip, hip, knee or elbow isometric muscle strength. The aim of this study was to examine the relationship between (Arthritis Impact Measurement Scales) AIMS, a health status questionnaire and handgrip strength, respiratory muscle strength, respiratory muscle endurance which are recorded as Maximum Inspiratory Pressure (MIP), Maximum Expiratory Pressure (MEP) and Maximum Voluntary Ventilation (MW). We also evaluated the relationship between AIMS and other clinical parameters. Twenty-five RA patients (17 women, 8 men) aged 25-71 participated in . . .this study. Patients had significantly reduced MIP, MEP, MVV and handgrip strength values compared to the healthy controls. Correlation analysis demonstrated statistically significant correlation between AIMS total score, AIMS dimensions and clinical parameters (duration of disease, duration of morning stiffness, Ritchie articular index, visual analogue scale, erythrocyte sedimentation rate, C-Reactive protein). Respiratory muscle strength or endurance and handgrip strength values were correlated with none of AIMS items. As a result, respiratory muscle strenth and endurance may be independent from functional andphysical status of RA patients. A 22-year-old woman presented with respiratory difficulty and quadriparesis two weeks after an upper respiratory tract intection. CT showed mild (Type I) rotatory atiantoaxial subluxation, but MRI demonstrated a severely contused and oedematous spinal cord at C2-3. The case was managed conservatively with collar, steroid and antibiotics. The outcome was excellent.

Genetic variability among Turkish pop, flint and dent corn (Zea mays L. spp. mays) races: Morphological and agronomic traits

32 Turkish corn accessions representing different climatic, geographic and topographic areas in Turkey were identified. Genetic variation and its pattern in three corn races (pop, flint and dent) included 32 accessions were investigated using 25 morphological and agronomic traits. Vegetative, ear and kernel related traits were significantly varied both among races and accessions within races. Variance component due to races and accessions within race were statistically significant, but the variance components due to accessions were generally made up large component of total variance and ranged from 17.17% in kernel length to 82.75% . . .in tassel length. There were also strong and mostly positive correlations between studied agronomic traits as well as between agronomic traits and isoenzyme loci. These correlations were explained by the traditional farming practices and enzyme specificity with flint accessions since the enzyme systems such as Alcohol dehydrogenase (locus 1), Malate dehydrogenase enzyme (loci 2-3) and 6-phospho-gluconate dehydrogenase (locus-1) are generally considered to be the characteristics of flint corn race. Canonical discriminant function analysis with twenty-five morphological and agronomic traits revealed that first two canonical discriminant variables explained 68% of total variation among accessions. Foucher's first dorsal metacarpal artery flap for thumb reconstruction: Evaluation of 21 cases

Background: Coverage of part of a soft tissue defect in the thumb, without bone shortening and without long-lasting immobilization in an inappropriate position leading to stiffness, is difficult to achieve. Objectives: To report our experience using Foucher's modification of the first dorsal metacarpal artery flap for thumb reconstruction in 21 cases. Methods: Foucher's flap is based on the neurovascular structures of the first dorsal metacarpal artery flap and radial nerve-sensitive branches on the dorsum of the second metacarpal and proximal phalanx. The cause of injury was work-related in all 21 cases. The patients' mean age was . . .37 (range 17-68 years), and mean follow-up was 19 months (range 12-31). Emergency surgery was performed in 13 patients with a time delay after injury of 4-12 hours. The minimum defect was 12×18 mm and the maximum 20×40 mm. Pedicular length was 55-95 mm. A skin bridge was left intact in 16 cases. In two cases of early postoperative venous congestion and flap loss, a cross-finger flap was performed as a salvage procedure. Results: Subjective satisfaction score was 8.37/10 (range 4-10); cold intolerance was experienced in 60% and dysesthesia in 33%. All except one patient are able to use their thumb in daily activity. Loss of mobility in the proximal interphalangeal joint of the index finger was less than 20 degrees. Semmes-Weinstein sensitivity evaluation score was 3.61-4.31 on the flap and 0-6.65 on the donor site. Two-point discrimination was 10.8 mm (range 8-20). Grip strength was reduced by 15% compared to the unaffected hand (hand dominance was not taken into consideration). Rehabilitation was not consistent as almost all the patients were living in another location. We have carried out an explicit closed-form determination of the inverse dielectric function for a single one-dimensional (1D) quantum wire embedded in a nonlocal 3D bulk semiconductor plasma and analyzed the coupled mode plasmon dispersion relation in the hydrodynamic model. Our analysis shows that the coupled 1D-3D plasmon is damped due to even small nonlocality of the 3D plasma. Also, there is a new nonlocal low frequency mode which does not exist for either the quantum wire or the nonlocal host in the absence of the other. © 2002 Elsevier Science Ltd. All rights reserved.

Effects of three different iron supplementations in term healthy infants after 5 months of life

The aim of this study was to investigate the effects of different doses of iron on haematological status of breastfed infants. One hundred and thirteen infants were randomized into four groups at 5 months of age. Iron supplementation was given at doses of 1 mg/kg/day, 2 mg/kg/day, and 2 mg/kg/every other day in the first three study groups, respectively, and the last group received placebo. Adhesions are a significant problem after tendon surgery. The effects of hyaluronic acid on adhesion formation of the tendo calcaneus were investigated in this study. Twenty Wistar rats were utilized. Both tendo calcanei were incised transversely, and then repaired. Hyaluronic acid (0.2 cc) was injected into peritendinous tissue on the right side, while the same amount of normal saline was injected to the left side as a control. The animals were sacrificed 40days after the experiment. Both the right and left tendon adhesions were evaluated both macroscopically and microscopically for the presence of adhesions (grading scale 0-4).
