Effects of statins in an indomethacin-induced gastric injury model in rats

Amaç: Statinlerin lipid düşürücü etkilerinin yanısıra ilave pleiotropik etkileri vardır. Bu çalışmada, statinlerin sıçanlarda indometazinle oluşturulan mide hasarı üzerine etkileri değerlendirildi. Gereç ve Yöntem: Hayvanlar sekiz gruba ayrıldı. Kontrol grubuna distile su, diğer gruplara omeprazol (30 mg/kg), atorvastatin (20-40 mg/kg), simvastatin (20-40 mg/kg) ve rosuvastatin (20-40 mg/kg) oral yolla verildi. Otuz dakika sonra, bütün gruplara yine oral yolla indometazin (25 mg/kg) uygulandı. Altı saat sonra, hayvanlar dekapitasyon ile öldürüldü. Her grup için ortalama ülser indeksleri hesaplandı. Bulgular: Ülser indeksleri kontrol grubunda 1.72±0.16, omeprazol grubunda 0±0.00, atorvastatin, simvastatin ve rosuvastatinin 20 ve 40 mg/kg dozlarında sırasıyla 4.28±0.39, 4.99±0.96, 1.72±0.73, 1.90±0.48, 1.85±0.26 ve 1.67±0.18 tespit edildi. Atorvastatin her iki dozda indometazinle oluşturulan ülser indeksini ve 40 mg/kg dozda erozyon skorunu anlamlı olarak artırmıştır. Simvastatinin 20 mg/kg dozu mononükleer lökosit infilrasyonunu inhibe etmesine rağmen, 40 mg/kg dozu hiperemiyi artırmıştır. Rosuvastatin 20 mg/kg dozda hem mononükleer lökosit hem de nötrofil infiltrasyonlarını, 40 mg/kg dozda ise sadece nötrofil infiltrasyonunu azaltmıştır. Sonuç: Mide rahatsızlığı olan hastalarda, statinler dikkatli kullanılmalıdır. Statin tedavisi mutlaka gerekiyorsa, atorvastatin kullanılmamasını ve diğer statinlerin de etkili en düşük dozlarda kullanılmasını tavsiye etmekteyiz. Distilled water (control group), omeprazole (30 mg/kg), atorvastatin (20 and 40 mg/kg), simvastatin (20 and 40 mg/kg), and rosuvastatin (20 and 40 mg/kg) were given orally (gavage). Thirty minutes later, indomethacin (25 mg/kg) was administered orally to all groups. Six hours later, the animals were sacrificed by decapitation. The mean ulcer indexes for each group were calculated, and the stomachs were evaluated histopathologically. Results: The ulcer indexes were as follows: control 1.72±0.16, omeprazole 0±0.00, and atorvastatin, simvastatin and rosuvastatin (at 20 and 40 mg/kg doses, respectively) 4.28±0.39, 4.99±0.96, 1.72±0.73, 1.90±0.48, 1.85±0.26, and 1.67±0.18. Atorvastatin significantly increased the indomethacin-induced ulcer index at both doses and the erosion score at 40 mg/kg dose. Although the 20 mg/kg dose of simvastatin inhibited mononuclear leukocyte infiltration, the 40 mg/kg dose induced hyperemia. Rosuvastatin did not decrease mononuclear leukocyte or neutrophil infiltrations at 20 mg/kg dose, and only neutrophil infiltration at the 40 mg/kg dose. Conclusions: In patients with gastric discomfort, statins must be used carefully. If statin therapy is needed, we recommend to avoid using atorvastatin and to use the other statins only in the minimum effective dose. Biliary cannulation facilitated by endoscopic clip assistance in the setting of intradiverticular papilla

Papilla bölgesinde duodenal divertikül olan hastalarda safra yollarının endoskopik kanülasyonu çok zor olabilmektedir. Yetmiş yaşında bir bayan hasta akut kolanjit ve koledokolithiazis tanılarıyla hastanemize başvurdu. Hastaya sıvı resüsitasyonu ve antibiyotik tedavisi başlandı. Endoscopic cannulation of the biliary tract may be challenging in cases with juxtapapillary, duodenal diverticula. A 70-yearold woman admitted to the hospital with the diagnosis of choledocholithiasis and acute cholangitis. She was placed on antibiotic treatment and parenteral fluid resuscitation. On endoscopic retrograde cholangiopancreatography, the papilla could not be cannulated because of the duodenal diverticulum. Various attempts at biliary cannulation failed. Endoscopic clips were used for evertion and fixation of a partially everted papilla from the diverticulum and for securing it in an orientation that allowed direct cannulation of the biliary tree. Cholangiography demonstrated multiple calculi within the choledochus. She was discharged from the hospital on the 9th day following endoscopic retrograde cholangiopancreatography in good condition. Gastric wall calcification in gastric cancer relapse: Case report

We present the case of a 53-year-old male with subtotal gastrectomy and gastrojejunostomy due to gastric cancer who later developed cancer relapse and diffuse plaque-like calcification in the residual gastric tissue. As far as we know, this is the first case in the English literature in whom gastric tumor calcification developed one year after gastric cancer operation. We also discuss possible mechanisms of gastric wall calcification in such cases.

Primary intestinal diffuse large B-cell lymphoma forming multiple lymphomatous polyposis

Multifokal ve atlamalı tutulum, primer gastrointestinal lenfomalar için oldukça nadir görülen bir gelişim paternidir. Klinik ve makroskopik özellikleriyle Crohn hastalığını taklit eden kaldırım taşı görünümü ile dikkat çeken bir ince barsağın multifokal diffüz büyük B hücreli lenfoma olgusu, klinik ve patolojik özellikleri ile beraber sunulmuştur. 25 yaşındaki erkek olgunun jejunoileal rezeksiyon materyalinde, arada sağlam görünümde mukoza alanları izlenen, en büyüğü 2,5 cm çapında olan, ülsere, serozayı da infiltre eden polipoid görünümde multipl lezyonlar izlendi. Mikroskopik incelemede, belirgin nükleollü, veziküler nükleuslu, ge . . .niş eozinofilik sitoplazmalı, pleomorfik, atipik karakterde lenfoid hücrelerden oluşmuş tümöral infiltrasyon gözlendi. Bu atipik hücrelerde vimentin, LCA, CD20, CD79a ile B hücre fenotipinde immün reaksiyon izlendi. Olguya, diffüz büyük B hücreli lenfoma tanısı verildi. Crohn hastalığı belirti ve bulguları ile başvuran hastalarda nadir de olsa bu antitenin olabileceği vurgulanmıştır. Multifocal and skip involvement is quite a rare developmental pattern for primary gastrointestinal lymphomas. A 25-year-old male patient with diffuse large B-cell lymphoma of the small intestine, with macroscopic features and clinical aspects imitating Crohn’s disease and attracting attention with cobblestone-like appearance, is presented herein together with the clinical and pathological features. Multiple ulcerated lesions were also observed infiltrating the serosa with polypoid appearance, 2.5 cm in largest diameter, within the resected jejunoileal specimen, which displayed patchy, healthy-appearing mucosal areas. In microscopic examination, a tumoral infiltration was observed comprised of pleomorphic, atypical lymphoid cells with abundant eosinophilic cytoplasm, marked nucleoli and vesicular nuclei. A B-cell phenotype immunoreaction was observed by vimentin, LCA, CD20, and CD79a in those atypical cells. Unsedated small caliber esophagogastroduodenoscopy: Can we trust this technique?

Small caliber endoscopes are one of the best examples of fantastic technological advancements in gastrointestinal endoscopy. First designed for pediatric patients in the 1970s, current small caliber videoendoscopes were used for unsedated transnasal gastrointestinal endoscopy after 1994. Nowadays, unsedated endoscopy can be successfully done using small caliber endoscopes via transoral or transnasal route in nearly 90% of cases. Several large studies have shown that small caliber endoscopy is feasible, safe and well-tolerated. These devices can decrease the potential risks of upper gastrointestinal endoscopy by eliminating the need . . .for sedation since these ultrathin endoscopes induce much less gag reflex or choking sensation in patients. Moreover, gastrointestinal endoscopy with small caliber endoscopes results in less sympathetic system activation as well as less oxygen desaturation compared to standard endoscopy, especially in aged, severely ill, bedridden patients. Nevertheless, there is no overall consensus on its cost effectiveness. Though indications are similar with standard endoscopy, small caliber endoscopy can be preferred in patients with gastrointestinal stenosis. Less common indications include transnasal endoscopic retrograde cholangiography and postpyloric feeding tube insertion. The esophagogastroduodenoscopy procedure with small caliber endoscopes is easy to perform, and there is generally no need for further training for this technique. However, the additional cost of equipment and some medicolegal and technical issues have resulted in the unpopularity of small caliber endoscopy in most countries other than France and Japan. However, sharing information about this technique and stressing its potential advantages can help in its widespread use in various countries including Turkey. Histopathologic evaluation of anti-ulcerogenic effect of montelukast in indomethacin-induced experimental ulcer model

Background/aims: The effects of anti-ulcerogenic drugs are dependent on the increase in prostaglandin production and reduction in leukotriene production in the gastric mucosa. Montelukast is an anti-asthmatic drug, a selective reversible cysteinyl leukotriene D4 receptor antagonist. In this study, we aimed to evaluate the anti-ulcerogenic effect of montelukast and to investigate the relationship between its anti-ulcerogenic effect and polymorphonuclear leukocyte infiltration in the gastric tissues. Materials and Methods: Male Sprague-Dawley rats were separated into five groups. Distilled water (control group), famotidine (40 mg/kg), . . . and montelukast (5, 10 and 20 mg/kg) were given orally (gavage). Thirty minutes later, indomethacin (25 mg/kg) was administered to all the groups. Six hours later, the animals were sacrificed by decapitation. The ulcer indexes for each stomach and the ulcer inhibition rates for each group were calculated, and the stomachs were later evaluated histopathologically (polymorphonuclear leukocyte infiltration). Results: Ulcer inhibition rates were as follows: famotidine 96.14% and montelukast 59.96%, 72.65% and 76.97% (5, 10 and 20 mg/kg, respectively). Montelukast (10 and 20 mg/kg) showed effects similar to those of famotidine histopathologically.
