A new laparoscopic technique for the placement of a permanent peritoneal dialysis catheter - The preperitoneal tunneling method

Background: Mechanical outflow obstruction and leakage from the exit site of the catheter are two common complications of continuous ambulatory peritoneal dialysis. To lessen these complications and to achieve immediate use of the catheter, we developed a new laparoscopic technique for catheter placement. Methods: A total of 12 consecutive patients with end-stage renal failure were included in this study between April 2003 and July 2003. The average age of the patients was 42.4 years (range, 37-72). Patients were excluded only if a serious risk for general anesthesia was found. Using two 5-mm ports and a 3.3-mm mini-laparoscope, a p . . .eritoneal dialysis catheter was passed through a preperitoneal tunnel before the tip of the catheter was introduced into the pelvis. Routine peritoneal dialysis was started immediately after the operation while the patients were still in the operating room. Results: The mean operating time was 18.6 min (range, 12-37). There was no operative morbidity. The mean follow-up period was 4.3 months (range, 3-7). No leakage of the dialysate liquid or outflow obstruction was observed during this period. Conclusion: The advantages of this method include accurate placement, preperitoneal fixation, and immediate use of the catheter for routine peritoneal dialysis. The aim of this study was to compare left ventricular (LV) functions by speckle tracking echocardiography (STE) in chronic kidney disease (CKD) patients in various stages and under different renal replacement treatments in order to evaluate possible differences between them. Peculiar thermodynamic properties of LJ(N) (N=39-55) clusters

The solid-liquid phase transitions of Lennard-Jones clusters LJ(N) (N=39-55) were simulated by a microcanonical molecular dynamics method using Lennard-Jones potential, and their thermodynamic quantities were calculated. The caloric curves of clusters (except N=42) have S-bend. To understand this behaviour, configurational and total entropies were evaluated, and dents on the entropy curves were taken as a sign of negative heat capacity. The heat capacities were evaluated for N=39-55 clusters using configurational entropy data. The potential energy distributions have bimodal behaviour for all clusters in the given range at the meltin . . .g temperature. The distinct melting behaviour of LJ(42) was explained by the topology of the potential energy surface by examining the isomer distributions at phase transitions for LJ(39)-LJ(55). The isomer distributions were found to be a useful way to interpret this behaviour and melting dynamics in general. Melting temperature, latent heat and entropy change upon melting values were reported and are consistent with literature values and values calculated from bulk thermodynamic properties. Tonnis triple pelvic osteotomy for Legg-Calve-Perthes disease

Purpose The aim of this study is to evaluate the efficacy of Tonnis triple pelvic osteotomy in patients with LCP disease. Methods Between 2007 and 2011, Tonnis triple pelvic osteotomy was performed for 43 patients with LCP, in our institute. During the follow-ups of patients with LCP the indications for the surgery were lateralization of the femoral head, insufficient femoral head coverage and subluxation of the femoral head. The mean age of patients at the time of surgery was 9.4 years. Results Before surgical intervention, according to Waldenstrom classification, three patients were in the necrosis stage (7 %), six patients in the . . . fragmentation stage (13.9 %), 16 patients in the re-ossification stage (37.2 %) and 18 patients in the remodeling stage (41.9 %). According to Herring classification, four patients were in group B (9.3 %), one patient in group B/C (2.3 %) and 38 patients in group C (88.4 %). After the operation, patients were evaluated with Stulberg classification, and good outcome (Stulberg I/II) was achieved in 23 patients (53, 5 %), fair outcome (Stulberg III) in 16 patients (37, 2 %) and poor outcome (Stulberg IV/V) was seen in only four patients (9.3 %). The mean value of CEA was 0.37 degrees pre-operatively and in the last follow-ups the mean value of CEA was 23.7 degrees. Conclusions We recommend triple pelvic osteotomy for patients with LCP when conservative methods are not successful.
