Treatment of intracoronary thrombus using tirofiban in a patient with normal coronary arteries

We report a 64-year-old Turkish man who presented with unstable angina pectoris. Coronary angiography revealed massive intracoronary thrombus, which completely occluded the distal part of the left circumflex coronary artery. The thrombotic segment and the rest of the coronary tree were free of atherosclerosis. The patient was treated with intravenous tirofiban, a glycoprotein IIb/IIIa inhibitor. A control angiography was performed one week later and showed total dissolution of the thrombus with tirofiban therapy. Copyright © 2004 by the Japanese Heart Journal.

Prediction of subclinical left ventricular dysfunction with strain rate imaging in patients with mild to moderate rheumatic mitral stenosis

Background: Left ventricular (LV) long-axis function evaluated by Doppler tissue echocardiography-derived strain rate (SR) imaging has been shown to be a useful index of LV systolic function; however, it has not been evaluated in patients with mitral stenosis (MS). We examined the LV long-axis function of patients with pure MS and normal global systolic function as assessed by LV ejection fraction. Method: In all, 30 patients (22 women; 45 ± 9 years) with mild to moderate MS (mitral valve area = 1.5 ± 0.3 cm2) and 28 healthy volunteers (20 women; 44 ± 10 years) were evaluated by both conventional and Doppler tissue echocardiography. . . . Two-dimensional Doppler tissue echocardiography was performed in the apical 4-chamber view in the septal and lateral wall on the mitral annular level. Peak systolic myocardial SR and end-systolic strain data were measured for both segments. Results: Peak systolic SR (1.2 ± 0.4% vs 1.8 ± 0.39%, P < .001) and end-systolic strain (10 ± 5 vs 25 ± 6 s-1, P < .001) were both significantly lower in patients with MS than in control subjects. Conclusions: Patients with MS had significantly impaired long-axis function evaluated by Doppler tissue echocardiography-derived SR imaging despite normal global systolic function. Severe depression with suicidal ideation associated with the use of the valsartan, an angiotensin receptor antagonist, is reported. The patient, who was unresponsive to antidepressant medication with two different groups, dramatically recovered following discontinuation of valsartan. Possible mechanisms and related issues of mood changes which caused by angiotensin II receptor antagonists, especially with valsartan are discussed. Clinicians should be aware that valsartan may cause mood changes.

This paper reports a 74 year-old-man with a hypertrophic cardiomyopathy associated with mid-ventricular obstruction and a secundum type atrial septal defect. Background/Aims: The aim of this study was to investigate atrial conduction times and left atrial mechanical functions, the noninvasive predictors of atrial fibrillation, in prediabetic patients with impaired fasting glucose (IFG) and impaired glucose tolerance (IGT). Methods: Study included 59 patients (23 males, 36 females; mean age 52.5 ± 10.6 years) diagnosed with IFG or IGT by the American Diabetes Association criteria, and 43 healthy adults (22 males, 21 females; mean age 48.5 ± 12.1 years). Conventional and tissue Doppler echocardiography were performed. The electromechanical delay parameters were measured from the onset of t . . .he P wave on the surface electrocardiogram to the onset of the atrial systolic wave on tissue Doppler imaging from septum, lateral, and right ventricular annuli. The left atrial volumes were calculated by the disk method. Left atrial mechanical functions were calculated. Results: The mitral E/A and E’/A’ ratios measured from the lateral and septal annuli were significantly lower in the prediabetics compared to the controls. The interatrial and left atrial electromechanical delay were significantly longer in prediabetic group compared to the controls. Left atrial active emptying volume (LAAEV) and fraction (LAAEF) were significantly higher in the prediabetics than the controls. LAAEV and LAAEF were significantly correlated with E/A, lateral and septal E’/A’. Conclusions: In the prediabetic patients, the atrial conduction times and P wave dispersion on surface electrocardiographic were longer before the development of overt diabetes. Atherosclerosis rates are higher in adult diabetic patients than normal population and is one of the major causes of death. The main reason of this is that serum lipid and lipoprotein levels are impaired, increasing the risk for atherosclerosis. For this reason, it is needed to investigate the alterations of serum lipid parameters in patients with impaired glucose tolerance (IGT) and impaired fasting glucose (IFG) and their effects on atherosclerosis and coronary artery disease (CAD). In this study, we measured lipid parameters, HbA1c, insulin and CRP levels in IGT and IFG patients and aimed to evaluate if they are under risk to dev . . .elop atherosclerosis and diabetes mellitus (DM). Serum triglyceride, insulin and CRP levels were found to be significantly high in IGT group, and serum triglyceride, HbA1c, insulin and CRP in IFG group, than the control group. Serum HDL-cholesterol level was found to be significantly low in both study groups than the control. While serum triglyceride level was higher in IFG group than IGT group, LDL-cholesterol and HbA1c levels were significantly higher in IGT group than IFG group. Also BMI values of control group were found to be significantly lower than both IFG and IGT groups. The results we obtained from this study showed that IFG and IGT patients are under high risk for atherosclerosis and CAD due to dyslipidemia and increased CRP levels and increased level of HbA1c may show that oxidant effects may be increased related with non-enzymatic glycosidation and related metabolic stress. Oxcarbazepine, 10 keto analogue of carbamezepine, is an antiepileptic drug used pri manly for the treatment of focal and generalized tonic clonic seizures in adults and children. Ox carbazepine has been observed to have a positive effect also on psychiatric diseases and trigeminal neuralgia. Oxcarbazepine like most of antiepileptic drugs, inhibits voltage dependent fast sodium channels. During oxcarbazepine treatment, side effects such as hyponatremia, headache, vertigo, somnolence, nausea, double vision, vomiting and ataxia can be seen. Spontaneous coronary artery dissection (SCAD) is relatively uncommon. It often occurs in healthy, middle-aged women without overt risk factors for atherosclerosis and is associated with the peripartum period. The pattern and severity of presentation are variable.
