Preoperative Evaluation of Lymph Node Metastases in Patients with Gastric Cancer: An Analysis of Imaging Methods

INTRODUCTION: Preoperative evaluation is necessary for the surgical treatment of gastric cancer (GC). Nonetheless, there is no single best diagnostic modality to predict lymph node metastases prior to surgery. The aim of this study was to analyze of the diagnostic utility of positron emission tomography-computed tomography (PET-CT) and CT for the preoperative evaluation of lymph node (LN) metastases in GC. METHODS: Eighty seven patients with a history of GC, who underwent gastrectomy and D2 LN dissection were investigated. Imaging test results and pathology reports were collected from the patients’ charts. RESULTS: There was no stat . . .istical differences between PET/CT and CT scans in regard to predicting LN metastases in GC patients (p>0.05). The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of PET/CT scans in predicting LN metastases were 55.56%, 54.55%, 55.26%, 75%, and 33.3%, respectively, versus 60.47%, 57.69%, 59.42%, 70.27%, and 46.88% for CT scans, respectively. However, combined PET/CT and CT showed better outcomes and specificity with a PPV of 80% and 88.9%, respectively. No patients or tumor factors were found to increase the accuracy of LN metastasis prediction using either PET/CT or CT scans. DISCUSSION AND CONCLUSION: The combination of CT and PET/CT scans increases the specificity and PPV. This increases the prediction accuracy of LN metastasis in GC patients compared to the use of each type of imaging modality alone. All the electrical demand of ship are provided by diesel generators (DG). Failure of generators due to breakdown can lead to the blackout of the ship which may suddenly cause serious damage to ship’s main and/or auxiliary engines, human health and marine environment or any structure at ship’s route. Thus, the planned maintenance and repairs of the generators must be done properly without any deficiency or delay. And even then, since the generators run continuously failures may often occur. A molecular dynamics study: Structures and thermal stability of PdmPt(13-m)Ag42 ternary nanoalloys

Structural optimization of ternary PdmPt(13-m)Ag42 nanoalloys was performed using the basin-hopping algorithm, and the Gupta many-body potential was adopted to model interatomic interaction. The optimization results show that all compositions have a structure based on icosahedron with a coreshell segregation. While the Ag atoms prefer to segregate to the surface, Pd and Pt atoms were located at the core of the cluster due to the higher surface and cohesive energy. The single platinum atom with the highest cohesive energy in Pd12Pt1Ag42 nanoalloy was located at the center of the cluster. Also in all other compositions except Pd13Ag42 . . ., Pd atoms occupy the second shell position of the icosahedron structure. We used classical molecular dynamics (MD) simulations in canonical ensemble conditions (NVT) to investigate the melting temperatures of ternary PdmPt(13-m)Ag42 nanoalloys with the interatomic interactions modeled by the same potential with optimizations. The icosahedral structures were taken as the initial configurations for MD simulations. We obtained caloric curves and Lindemann indexes to investigate the melting transitions. The simulation results showed that varying the composition gives rise to a fluctuation in melting temperatures. The highest melting temperature belongs to the Pd9Pt4Ag42 nanoalloy cluster within the other compositions. However, the relative stability investigation indicates the Pd8Pt5Ag42 nanoalloy cluster as the most stable composition. OBJECTIVE: The aim of the study was to examine the efficacy of electroconvulsive therapy (ECT) combined with antipsychotic (AP) medication on symptom profile in patients with a diagnosis of schizophrenia who had received acute psychiatric inpatient treatment. METHODS: In this prospective study, patients were evaluated for inclusion in the study who were diagnosed with schizophrenia according to DSM-IV diagnostic criteria and were to receive ECT. The patients were evaluated using the Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of Positive Symptoms (SAPS), Scale for the Assessment of Negative Symptoms (SANS), and C . . .linical Global Impression - Improvement (CGI-I) sub-scale before the first session ECT, once following every two subsequent sessions and after the final session. RESULTS: The patients showed significant improvements in BPRS scores at each evaluation compared with their scores at baseline, and a significant clinical improvement was found on the CGI-I sub-scale at the end of treatment. Across all SAPS sub-scores, significant decreases were found, and the symptoms related to hallucinations and positive formal thought disorder showed the most rapid response to treatment. Across all SANS sub-scores, significant decreases were found, and affective flattening or blunting symptoms responded most rapidly to treatment. CONCLUSION: One of the most important findings in the present study of hospitalized patients with acute schizophrenia was the good response to treatment, which provided significant improvements in both positive and negative symptoms. The most rapid response to treatment was found for hallucinations, positive formal thought disorder, and affective flattening or blunting symptoms. The most important limitation of our study may be the small number of cases. We investigated ground-state nuclear structure properties of even and odd 103-140Sb, 105-143Te, 107-145I, 109-148Xe and 112-151Cs nuclei via the axially deformed solution of Skyrme Hartree-Fock Bogoliubov (HFB) based on the transformed harmonic oscillator (THO) and the harmonic oscillator. Examination of foramen tympanicum: An anatomical study using cone-beam computed tomography

Background: The foramen tympanicum (FT) is an osseous dehiscence of the temporal bone and usually closes by the age of 5 years. The foramen is located posteromedial to the temporomandibular joint and anteroinferior of the external auditory canal. The aim of this study is to define the prevalence, location and size of the foramen. Materials and methods: We retrospectively examined 200 cone-beam computed tomography (CBCT) images (400 ears). We used a CBCT dental imaging system (GALILEOS, Sirona Dental Systems, Bensheim, Germany) working at 15-30 mA and 98 kV. We noted size and location (unilateral and bilateral) of the present FT. Res . . .ults: Foramen tympanicum was determined in 11.5% of 200 patients (they had FT at least on one side of the head). This was bilateral in 5 (2.5%) patients. Prevalence of the FT was significintly higher in females (8%) than in males (3.5%). FT was found more frequently on the left side (7.5%) than on the right side (4%). Mean axial diameter was 1.13 mm (range 0.23-4.43 mm), and mean sagittal diameter was 1.44 mm (range 0.22-3.99 mm). Conclusions: Considering that FT was present in 11.75% of patients, radiologists and clinicians should be aware of the possible presence of this defect. It is known that this developmental dehiscence may cause herniation of temporomandibular joint, formation of salivary otorrhea, and spread of tumour or infection to the infratemporal fossa from external auditory canal. I utilize disclosed information in pre-issue prospectuses and valuation reports as well as post-issue material event files to document that: initial public offerings implementing price support have lower initial and short-term returns, and larger optimistic valuation bias relative to offerings without price support. Electrodeposition of Si–DLC nanocomposite film and its electronic application

In this study, a silicon doped diamond-like carbon (DLC) nanocomposite film was deposited electrochemically and it was used to fabricate an Ag/Si–DLC/p-Si metal–interlayer–semiconductor (MIS) Schottky diode. Methanol (CH3OH) was used as carbon and tetraethoxysilane (Si(OC2H5)4) as Si source, respectively. Morphology of the film was observed by scanning electron microscopy and continuous surface with numerous lumps spread randomly was observed. Structural and chemical composition analyses of the film were carried out by Raman and X-ray photoelectron spectroscopy. Typical D and G bands of DLC films were not observed in the Raman spect . . .rum due to abundance of Si–C bonds. Silicon incorporation promoted the film deposition and increased sp3 bonds in the film. Current–Voltage (I–V) measurement was conducted to obtain parameters of the MIS diode. The diode exhibited a good rectifier behavior with ~ 103 rectification ratio. In the forward bias semi-logarithmic I–V plot, two linear regions with different slopes were observed. Such behavior was modelled by two parallel diodes and attributed to different conduction mechanism. The main electrical parameters, such as barrier height, ideality factor and series resistance were calculated by I–V and Cheung–Cheung methods. This paper explores the explanations for, and consequences of, the early appearance of food production outside the Fertile Crescent of Southwest Asia, where it originated in the 10th/9th millennia cal BC. We present evidence that cultivation appeared in Central Anatolia through adoption by indigenous foragers in the mid ninth millennium cal BC, but also demonstrate that uptake was not uniform, and that some communities chose to actively disregard cultivation. Adoption of cultivation was accompanied by experimentation with sheep/goat herding in a system of low-level food production that was integrated into foraging practices rather t . . .han used to replace them. Furthermore, rather than being a short-lived transitional state, low-level food production formed part of a subsistence strategy that lasted for several centuries, although its adoption had significant long-term social consequences for the adopting community at Boncuklu. Material continuities suggest that Boncuklu’s community was ancestral to that seen at the much larger settlement of Çatalhöyük East from 7100 cal BC, by which time a modest involvement with food production had been transformed into a major commitment to mixed farming, allowing the sustenance of a very large sedentary community. This evidence from Central Anatolia illustrates that polarized positions explaining the early spread of farming, opposing indigenous adoption to farmer colonization, are unsuited to understanding local sequences of subsistence and related social change. We go beyond identifying the mechanisms for the spread of farming by investigating the shorter- and longer-term implications of rejecting or adopting farming practices. © 2018 National Academy of Sciences. Critical-care nurses' pain experiences and the prognostic factors

Objective: The objective of this study was to determine the emergence of pain in critical-care nurses and the prognostic risk factors. Design: This study is a cross-sectional descriptive research. Setting and Subjects: This study was conducted with the participation of 111 critical-care nurses in Zonguldak province, Turkey. Interventions: The data were collected using a survey form and the Cornell Musculoskeletal Discomfort Questionnaire between August and November 2015. The data were evaluated using frequency, average, Pearson correlation analysis, ?2, logistic regression, and odds ratio tests. Results: The critical-care nurses wer . . .e found to experience pain mostly in their lower backs (88.3%), upper backs (77.5%), right (76.6%) and left (78.4%) feet, necks (73.9%), and most infrequently in the right (28.8%) and left (28.8%) lower arms. Changing bed linens while the patient remained in bed and lifting, pulling, or pushing heavy materials caused those nurses who felt despondent and tired to feel significant pain in the shoulder, neck, upper arm, wrist, and knee (P <.05). Conclusions: These results were significant for placing emphasis on maintaining body mechanics while caring for patients, providing appropriate environmental conditions, and ensuring ergonomics to preserve the health of critical-care nurses who work in hospitals.
