Anjiyomiyolipoma böbrekte, retroperitoneal hemoraji yapabilen, mikroskopik olarak damar yapıları, düz kas ve yağ dokuları içeren benign tümöral lezyondur. Lenfanjiyomiyomatozis ise; akciğerleri difüz tutabilen, respiratuar yetmezlik, spontane pnömotoraks veya şilöz plevral efüzyon izlenebilen lezyondur. Mikroskopik olarak lenf damarları ve düz kas elementlerinin karışımının proliferasyonları ile karakterizedir. Her iki lezyon da az rastlanan bir sendrom olan tuberoskleroz kompleksinin komponenti olarak izlenebilir. Olgumuz göğüs ağrısı, nefes darlığı şikayeti olan 41 yaşında bayan hastadır. Toraks tomografisinde akciğer parankiminde . . ., ince duvarlı hava kistleri, abdominal tomografide böbreklerde büyüme ve yağ dansitesinde alanlar içeren lezyonlar belirlenmiştir. İntraabdominal kanama nedeniyle sağ nefrektomi uygulanmıştır. Mikroskobide, böbrek parankimindeki infiltratif tümörün matür lipomatöz, miyomatöz komponentler ve konjesyone vasküler yapılardan oluştuğu izlenmiş, immünohistokimyasal incelemede tümör hücrelerinde HMB-45 ile reaksiyon gözlenen olguya anjiyomiyolipoma tanısı konulmuştur. Akciğer lezyonlarının mikroskobik incelemesinde, kistik dilatasyonlu, düz kas hücreleri içeren lenf damarları izlenmiştir. Trikrom ile lenf damarları duvarında düz kas yönünde boyanma gözlenmiş ve lenfanjiyomiyomatozis olarak değerlendirilmiştir. Olgumuz postoperatif 16. günde gelişen kardiyopulmoner arest nedeni ile eksitus kabul edilmiştir. Angiomyolipoma is a benign tumoral lesion of the kidney which microscopically contains fat, smooth muscle and blood vessels, and may result in retroperitoneal hemorrhage. Lymphangiomyomatosis is a lesion that may lead to diffuse involvement of lungs and cause respiratory failure, spontaneous pneumothorax or pleural effusion. It is characterized by proliferation of a mixture of lymphatic and smooth muscle. Both lesions may be components of tuberous sclerosis, an uncommon syndrome. Our case is 41-year-old female presented with chest pain and dyspnea. Thin walled air cysts were determined in lung parenchyma by thorax tomography and images of enlarged kidneys and mass lesions including areas of adipose density were supplied by abdominal tomography. Right nephrectomy was performed because of intraabdominal hemorrhage. Infiltrative tumor microscopically composed of mature lipomatose, myomatose components and congested blood vessels, and reactive for HMB-45 [Melanoma (gp100) Ab-3 (clone HMB45+HMB50), Neomarkers] was diagnosed as angiomyolipoma. Microscopic evaluation of the lung lesions revealed cystic lymphatic containing smooth muscle cells. The tumor with presence of smooth muscle in the walls of lymphatic shown with trichrome and was diagnosed as lymphangiomyomatosis. Objective: Cardiorenal syndrome (CRS) describes a dysregulation of the heart and kidneys affecting each other. Recently hemodialysis treatments were used more frequently. Aim was to analyze the effects of conventional diuretic and UF treatments. Material and Methods: Thirty-four Type 4 CRS diagnosed patients were included. Baseline characteristics were recorded. Echocardiography measured at the admission and at the end of the treatment. Results: The mean age 67.4±9.3 (51-93) years and follow-up period were 15.9±11.5 months. The patients were grouped as diuretic group, n=12 and UF group, n=22. At the beginning mitral valve A wave, bl . . .ood urea nitrogen and creatinine values were higher in the UF group while creatinine values were higher in the UF group compared to diuretic group at the end of the study. Although basal ejection fraction (EF) values were not different, it was higher in the UF group at the end of the study (42.38±12.70 % and 29±3.67 %, p 0.05). Conclusion: In Type 4 CRS, mortality and hospital admissions were not reduced by UF treatment but cardiac function assessed by EF was significantly improved suggesting this therapy to be beneficial in appropriate patients Daha fazlası Daha az

Neuroprotective effects of estrogens and progesterone have been widely studied in various experimental models. The present study was designed to compare possible neuroprotective effects of 17alpha-estradiol, 17beta-estradiol, and progesterone on oxidative stress in rats subjected to global cerebral ischemia. Global cerebral ischemia was induced in ovariectomized female rats by four vessel occlusion for 10 min. Following 72 h of reperfusion, levels of malondialdehyde (MDA, oxidative stress marker), and reduced glutathione (GSH, major endogenous antioxidant) were assessed in hippocampus, striatum and cortex of rats treated with either . . . 17alpha-estradiol, 17beta-estradiol, progesterone or estradiol + progesterone beforehand. Steroid administration ameliorated ischemia-induced decrease in GSH and increase in MDA levels. Herpes zoster is an infectious disease caused by varicella-zoster virus that may occur sporadically at any age. We report on two patients with herpes zoster who received chemotherapy for breast cancer. Both patients were immunocompromised and received filgrastimtherapy for themanagement of neutropenia. Zoster occurred during filgrastim therapy but the symptoms were alleviated rapidly in the course of therapy. We conclude that granulocyte colony-stimulating factor therapy helped symptom alleviation and accelerated the recovery fromherpes zoster in our chemotherapy-treated patients. Transverse testicular ectopia is rarely associated with incarcerated inguinal hernia. A 14-month-old male complaint of irreducible inguinal hernia due to transverse testicular ectopia is reported. The clinical and operative findings and treatment options are discussed. It is thought that surgeons who frequently repair inguinal hernias should be aware of the appropriate surgical management options available to them when this condition is unexpectedly identified during inguinal exploration. © 2008 Tatli and Numanoglu; licensee BioMed Central Ltd.

The incidence of numerous head and neck tumours is a known issue though bilateral synchronous tonsillar carcinoma reports are so uncommon that only 20 cases were found in a literature review. Most of these patients were treated with bilateral tonsillectomy followed by adjuvant radiotherapy. We report, to our knowledge, the first case of bilateral synchronous tonsillar squamous cell carcinoma treated only with chemoradiotherapy without tonsillectomy.

Objectives: To determine whether angiotensin and endothelin have any role in testicular ischemia reperfusion injury by investigating the effects of the angiotensin converting enzyme inhibitor enalapril, selective non-peptide angiotensin-II type I blocker losartan and dual endothelin receptor blocker bosentan. Methods: Rats were anesthetized with thiopental sodium (50mg/kg i.p.) before the operation. The left testicular artery and vein of rats were occluded for 1h; before the bilateral orchiectomy, the organ was allowed to reperfuse for 3h or 24h. Enalapril (20mg/kg i.p.), losartan (30mg/kg i.p.), bosentan (10mg/kg i.p.) or vehicle ( . . .saline) were given 30min before reperfusion. Malondialdehyde level was measured in testicular tissue after 3h of reperfusion. Histological examination was carried out after 24h of reperfusion. Results: Ischemia reperfusion caused a significant increase in malondialdehyde level of ipsilateral testis, and histopathological injury in both ipsilateral and contralateral testes. Enalapril, losartan and bosentan treatments prevented the ischemia reperfusion-induced augmentation in malondialdehyde levels. Only bosentan treatment ameloriated ischemia reperfusion-induced histopathological alterations. Conclusions: Endothelin might play a more important role in pathogenesis of testicular ischemia reperfusion injury when compared with angiotensin. © 2011 The Japanese Urological Association Daha fazlası Daha az

Spontaneous coronary artery dissection (SCAD) is a rare cause of myocardial ischemia. Multivessel SCAD is much rarer than single vessel involvement and acute coronary syndrome remains the most common clinical presentation of a patient with SCAD. It predominantly occurs in association with atherosclerosis or in the absence of atherosclerosis. We, hereby, present a case of spontaneous multivessel coronary artery dissection in a 35-year-old male patient presenting with chest pain and ventricular tachycardia following emotional stress and discuss the etiology and treatment options. © 2011 Wiley-Liss, Inc. A congenital cystic mass was detected at the suprasternal notch of a seven-month-old male infant. After radiologic examinations including ultrasonography, computed tomography, and magnetic resonance imaging, the cyst was excised and diagnosed as a dermoid cyst. Dermoid cysts of the head and neck are rare lesions, but a midline location is characteristic for these congenital masses. To our knowledge, only two reports have been published, which were similar to our case in localization. Dermoid cysts should be included in the differential diagnosis of midline cysts.
