Simultaneous occurrence of different follicular neoplasms within the same thyroid gland [Aynı tiroid bezinde farklı foliküler hücre kökenli neoplazilerin birlikteliği]

Purpose: Neoplasms of the thyroid gland are classified according to the cells they originate from and commonly develop from cells of follicular origin. The most common differentiated thyroid cancers (DTC) are papillary and follicular carcinomas. Coexistence of two different histological types of primary follicular thyroid neoplasm is a rare condition. There are previous reports of concomitant medullary and papillary thyroid cancers. However, there is scarce data about the simultaneous occurrence of the two different histological types of primary follicular thyroid tumors and this is the first study on that subject. Material and Meth . . .od: From January 2007 to September 2014, our institutional database was reviewed for patients who underwent thyroid surgery for various indications. Medical records and cytopathology reports of those patients were examined retrospectively. Simultaneous neoplasms of follicular origin were noted. Results: A total of 3.700 patients were operated. Histopathological examination revealed a benign pattern in 2.686 (73%) patients and a malignant pattern in 1.014 (27%) patients. Among the patients with the diagnosis of DTC, only 20 (1.9%) had a concomitant neoplasm within the same thyroid gland. Discussion: Such simultaneous tumors may be a part of a familial tumor syndrome or an unidentified novel gene mutation playing role in the pathogenesis of more than one type of tumor. Based on the current evidence, the synchronous occurrence of those neoplasms in a given patient is likely coincidental in the literature. Objective: Hepatitis B virus (HBV) is a public health problemowing to communicable and widespreant. In addition to It is important issue because of preventable and prevent to people from contaminated persons. Transmission of HBV may be connected to transmission carrecteristics such as horizontal, vertical, sexual and blood, priority. Aim of this study is to determine the possible route of HBV transmission throughout a family setting. Material and Methods: 173 members (29 spouses, 82 offspring, 11 mothers, 11 fathers and 40 siblings) of 40 index cases (HBsAg carriers and chronic active hepatitis cases) attending the Harran University . . . Medical Faculty Hospital between Augst 2006 and March 2007 were screened prospectively for markers of HBV by using an ELISA. The chi-squared and Logistic regression analyses were used for statistical analysis. Results: Index cases was 18.8% of all family members, while HBsAg prevalence is 15.0% among family members except index cases. Among family members, the highest prevalences of antiHBc were in the mothers (54.5%) and sisters (45.5%), brothers (33.3%), sons (32.6%)and wifes (31.3%), the least prevalences of antiHBc were husbands (7.7%), fathers (18.2%), and doughters (22.2%) of all index cases. Transmission of HBV to both the mothers and sisters were positive was high, Transmission of HBV to fathers was low in all families. There was significant difference in cases where only the mother and husbands of index cases for HBV transmission (p= 0.012). In the logistic regression model; living with chronic hepatitis B index case (OR= 1.92; CI= 1.03-3.57), to be illiterate (OR= 6.81 CI= 1.27-22.13), born at home with traditional methods (OR= 3.70; CI= 1.15-11.87) and born at home with the help of midwife (OR= 3.44; CI= 1.47-7.91) were higher risk for hepatitits B transmision. Conclusion: The family members of index cases with inactive HBV carriers, particularly in the case of chronic active hepatitis have a high risk of transmission. Objective: In many clinical and experimental trials, researchers assess the effect of treatment by measuring the value of a continuous variable before and after the treatment. If there is an imbalance in baseline values between groups, some statistical comparisons may result with mistakes in estimation of the treatment effect. The aim of this study was to explain which statistical methods were more suitable in the estimation of the treatment effect when there was an imbalance for the baseline values between groups. Material and Methods: Different statistical methods, which are used in estimation of treatment effects, were briefly ex . . .plained and were applied to a hypothetical data set, which had significant differences between groups according to baseline values of the related variable. In addition, a limited simulation study for several conditions was carried out to determine suitable statistical methods. Results: Baseline values were different between two groups and correlation was low between baseline and follow up values of related variable in each group for hypothetical data set. In this condition, comparison of simple differences between baseline and follow up values was the best method for the estimation of treatment effect. In the simulation study, the power of the test for simple differences was higher (85%) than the value in the analysis of covariance (40%) when correlations were low and sample sizes were small in each group. Moreover, the powers of these two tests were high and similar to each other, when sample sizes were moderate. When the correlation was high, the powers of both tests were high in both small and moderate sample sizes. Conclusion: The presence of a significant difference should be sought between groups according to baseline values of the related variable even though groups are randomly assigned. In addition, the degree of the correlation between baseline and follow up values should be taken into consideration. When significant differences exist between baseline values and the correlation is low, we suggest that the classical methods should be used to determine the significance of the effect; however, when the correlation is high, covariance analysis is a suitable method. Objective: Drug-induced rhabdomyolysis is the most common cause of disorders leading to acquired muscular inflammation. The aim of this study is to investigate rhabdomyolysis frequency in patients admitted to the emergency room (ER) due to one or different types of multiple drug ingestion with suicidal intentions. Mole rats have adapted to living underground and have a wide range chromosomal variation in Turkey, ranging between 2n = 36 and 2n = 60. This study was performed on the subterranean mole rats of the Nannospalax nehringi (Nehring, 1898), sampled around Çanki{dotless}ri{dotless} and Çorum provinces in central Anatolia, and the karyotypes of 91 specimens across 38 localities were analyzed. Different chromosomal forms that have the same 2n values were assigned letters according to their geographic locations in Turkey: C for central forms, N for northern forms, S for southern forms, E for eastern forms, and W for western forms. We present two cases of Kartagener syndrome with clinical and radiological findings. Routine thorax and paranasal sinus as well as high-resolution CT examinations were performed during the diagnosis of two patients suffering from extensive mucus and persistent cough. Objective: We investigated the effects of different intensities of sinusoidal magnetic fields of daily life exposures on the peripheral nerve regeneration daily life exposure. The effects of magnetic field (50 Hz frequency and 1, 2, 3 mT intensities) treatment on sciatic nerve regeneration were studied in a model of crush injury of the sciatic nerve of rats. Material and Methods: Prior to crush injury, compound muscle action potentials were recorded for all rats. Then, the sciatic nerve was compressed for 15 sec with a jewelers forceps (no: 5). One rat group was kept as control and the remaining were assigned as 1, 2 and 3 mT magnet . . .ic field treatment groups. The magnetic fields 1, 2 and 3 mT were applied to 30 rats for 4 hours per day for a week using Helmholtz coil systems. The control group was included in the system for 4 hours per day for a week while the system was turned off. Results: In terms of nerve regeneration, 3- and 7-days treatments of different magnetic field intensities had no statistically significant effect on amplitude and conduction velocity of correspound muscle action potential. Conclusions: The intensities and frequency of magnetic fields used in this study had no effect on conduction and excitability of nerves and consequently did not influence the nerve regeneration. Behçet disease is a chronic, inflammatory, multisystemic vasculitis mainly characterized by orogenital ulcers. It is mainly observed in Turkey, other Mediterranean areas, and Japan. Viral, bacterial, genetic, immunologic, and environmental factors have been implicated in the pathogenesis of the disease. Although disease is multisystemic, cardiac involvement is rare and often associated with poor prognosis. Hypercoagulopathy is one of the important manifestation of the Behçet disease. Thromboembolic events such as myocardial infarction, stroke and pulmonary embolism risk is higher in patients with Behçet compared to normal population . . .. Right ventricular thrombus is a quite rare. In this paper we present a 26-year-old male patient who presented with pulmonary embolism and was subsequently diagnosed with a right ventricular thrombus and completely recovered after treatment. Objective: Melatonin, a hormone produced by the pineal gland, has been suggested to protect against development of hypertension and atherosclerosis. In this study, the effects of long-term melatonin deficiency for twelve months after pinealectomy on the ?-adrenergic-contractions induced by phenylephrine, endothelium-dependent relaxation responses to acetylcholine and the morphological changes in the rat thoracic aorta were studied. Material and Metods: Rats were pinealectomized twelve months before the beginning of the vasomotor studies. Rings of arteries were mounted in isolated tissue baths for the measurements of isometric contra . . .ctile force. The contractile responses to phenylephrine and endothelium-dependent relaxation responses to acetylcholine in the vessels were evaluated. Endothelial function was evaluated by vascular relaxation to acetylcholine. Histological examinations demonstrated the alterations of tunica media in the vessels of pinealectomized rats. Results: Thick and thin areas were observed in the transverse sections of vessels and the ratio of the widest media thickness to the narrowest was found significantly increased in pinealectomized group (2.85 ± 0.56) when compared to the control group (1.65 ± 0.10). In addition, ?-smooth muscle actin and elastic lamellae staining of the media were attenuated in pinealectomized rats. Al-though contractile responses of vessels to phenylephrine in pinealectomized rats were lower than control group, significant difference was found for only one concentration (3x 10-8 mol l-1) of phenylephrine. There was no difference between the relaxation responses to acetylcholine in pinealectomized and control groups. Conclusion: These results show that long-term melatonin deficiency may cause some morphological changes in the tunica media of vessels. Anti-inflammatory effects of L-type calcium channel blockers

Objective: Calcium ion has an important role in the synthesis and release of chemical mediators during inflammation. The aim of this study was to examine the anti-infiammatory effects of L-type calcium channel blockers on acute and chronic inflammation models in rats and also to examine the effects of nicardipine on capillary vascular permeability in rabbits. Material and Methods: Effects on acute phase of inflammation of verapamil, diltiazem and nicardipine (10 mg/kg doses) were compared with diclofenac sodium (25 mg/kg) in histamine-induced paw edema model. After measuring the right hind paw volumes of rats, drugs were injected in . . .traperitoneally 30 minutes later. Paw edema was induced by 0.1 mL subplantar histamine (0.1%) injection to the same paw. Subsequent paw volumes were measured at 30 minutes intervals. Effects of the drugs on the chronic phase were tested with cotton pellet granuloma method in rats, and effect of nicardipine on capillary vascular permeability was examined with hyaluronidase test in rabbits. Results were compared with control groups. Results: In acute inflammation model, after the histamine injections, maximal paw edema was observed at 30 min. While verapamil and diltiazem did not have an anti-inflammatory effect, nicardipine and diclofenac sodium showed significant anti-inflammatory activities of 63.77% (p= 0.000) and 42.93% (p= 0.002), respectively. While verapamil and diltiazem had similar effects to those of the control group and to each other (p> 0.05), nicardipine had the most potent activity (p< 0.05). Also, nicardipine (3 mg/kg) significantly reduced hyaluronidase-induced capillary permeability (p= 0.000). Calcium channel blockers did not show anti-proliferative effects oi the chronic phase of inflammation, Conclusion: Since nicardipine, a calcium channel blocker significantly inhibited histamine-induced acute inflammation and the dissemination area of hyaluronidase, its anti-inflammatory effect may be due to its higher vaso-selectivity on peripheral vascular smooth muscles and on preventing the increase of vascular permeability.
