OBJECTIVE To investigate the value of anatomic and blood-flow measurements of the prostate via ultrasound in identifying medical treatment response of patients with lower urinary tract symptoms due to benign prostatic enlargement. A secondary objective of the study was to compare transabdominal-transrectal measurements. MATERIALS AND METHODS Male patients (age range 50-80) with lower urinary tract symptoms who applied to our outpatient clinic to be diagnosed with benign prostatic enlargement that were eligible for medical management were included in the prospective study. At baseline evaluation, all patients underwent International Prostate Symptom Score (IPSS), prostate-specific antigen (PSA), transabdominal and transrectal ultrasound [prostate volume (PV), post-void rezidu and intravesical prostatic protrusion (IPP)], and prostate subcapsular resistive index value measurement. Follow-ups were carried out 1 and 3 months after treatment. At these time-points, patients were assessed with IPSS and uroflowmetry. RESULTS IPP were identified to be negatively correlated with baseline Qmax values (P =.001, R = -0.485) and positively correlated with PSA (P = .013, R = 0.353) and PV (P = .003, R = 0.420). PV was related with resistive index (P = .026, R = 0.318) and PSA (P = .001, R = 0.619). Subsequent to medical treatment on the follow-ups, an overall improvement in the IPSS and Qmax was identified (P = .001). However, a negative correlation was identified in the change that occurred between IPSS and IPP (P = .004, R = -0.410). Other parameters were not correlated (P > .05) with the change in IPSS and Qmax. Transabdominal and transrectal measures were similar (P = .001). CONCLUSION This study has shown that increased IPP values are associated with lower response to alphareceptor specific management. Objective: To determine the location and radiological characteristics in children with abdominal hydatid disease (HD). Materials and methods: Thirty-one children (average age: 7.2 years) with abdominal HD were studied. The number, location, diameter and internal architecture of the cysts were assessed with abdominal ultrasonography (US) and computed tomography (CT). Density measurements and enhancement patterns were determined on CT. Results: Twenty-one children had hepatic HD. The remaining 10 children had both hepatic and extrahepatic cysts. There were splenic cysts in five children, peritoneal cysts in two children and combined s . . .plenic and peritoneal cysts in three children. The most common site of the cysts was the liver (64%), followed by the spleen (20%) and the peritoneal cavity (16%). The seven intraabdominal cysts, which were not detected by US, were 20 min or less in diameter. Conclusion: CT may demonstrate additional small intrahepatic or unsuspected extrahepatic cysts. Although rare, splenic or peritoneal hydatidosis should be included in the differential diagnosis of a cystic splenic or peritoneal lesion. Familiarity with atypical locations of HD may be helpful in making a prompt, accurate diagnosis. We think that in particular patients, especially those who had diagnostic problem and who are under surgical planning, CT should be performed additionally. (C) 2004 Elsevier Inc. Purpose: Ischemia-reperfusion injury is one of the consequences of tourniquet application for extremity surgery. The aim of the study was to establish the effect of dexmedetomidine on the acute lung injury following lower extremity experimental ischemia-reperfusion model in rats. Methods: Twenty-eight Wistar-Albino breed rats were recruited after Ethics Committee approval and allocated into 4 groups, each with 7 subjects. Group 1 (SHAM) received only anesthesia. Group 2 (IR) had experienced 3 h of ischemia and 3 h of reperfusion using left lower extremity tourniquet after anesthesia application. Groups 3 (D-50) and 4 (D-100) had und . . .ergone the same procedures as in the Group 2, except for receiving 50 and 100 mg.kg(-1), respectively, dexmedetomidine intraperitoneally 1 h before the tourniquet release. Blood samples were obtained for the analysis of tumor necrosing factor-alpha and interleukin-6. Pulmonary tissue samples were obtained for histological analysis. Results: No significant difference regarding blood tumor necrosing factor -alpha and interleukin-6 values was found among the groups, whereas pulmonary tissue injury scores revealed significant difference. Histological scores obtained from the Group 2 were significantly higher from those in the Groups 1, 3 and 4 with p-values 0.001 for each comparison. Moreover, Group 1 scores were found to be significantly lower than those in the Groups 3 and 4 with p-values 0.001 and 0.011, respectively. No significant difference was observed between the Groups 3 and 4. Conclusion: Dexmedetomidine is effective in reduction of the experimental ischemia-reperfusion induced pulmonary tissue injury in rats, formed by extremity tourniquet application. Objective: To compare the factors affecting sexual function in premenopausal women with type 2 diabetes using the female sexual function index (FSFI). Design: Cross-sectional study. Setting: Premenopausal women with type 2 diabetes and healthy volunteers in an academic research environment. Patient(s): Sixty-two type 2 diabetic women were compared with 50 controls. The FSFI scores of the patients were compared according to age, duration of diabetes, marriage, socioeconomic status, parity, menstruation regularity, body mass index (BMI), and smoking. Intervention(s): None. Main Outcome Measure(s): Female sexual function index scores. . . .Result(s): The mean FSFI score in diabetics was lower than in controls. Menstrual irregularity and duration of diabetes affected female sexual function unlike age, BMI, socioeconomic status, duration of marriage, or parity. Regularly menstruating women had higher FSFI scores (27.1 +/- 6.7) than irregularly menstruating women (20.7 +/- 7.4). Type 2 diabetics were divided into three groups according to duration of diabetes. The women having diabetes longer than 10 years had the worse sexual function. Also, smoking affected inversely female sexual well being. Conclusion(s): Menstrual disturbance, smoking, and longer duration of diabetes (more than 10 years) impair FSFI scores in premenopausal women with type 2 diabetes. (Fertil Steril (R) 2010;94:1840-3.
