This study aimed to compare the results of AP screws, PA screws and posterior buttress plate used in posterior malleolar fixation of trimalleolar fractures. In this study, the data of 104 trimalleolar fracture cases treated surgically between October 2011 and January 2014 were extracted from hospital records. Patient demographics (age, gender, body mass index), the presence of syndesmotic injury, fracture type (according to the Lauge-Hansen classification), the size of posterior malleolar fragment (measured on lateral radiographs) were recorded. The patients requiring postoperative care for at least a year were invited to attend an . . .evaluation of functional and radiological outcomes. At the final follow-up examination the functional evaluation was made by using AOFAS scores, VAS scores during walking, and dorsiflexion restrictions as compared with the unaffected side. As for the radiological evaluation, the patients were assessed according to the presence of a gap or step by the direct use of graphies following the fixation and with regard to the osteoarthritic development in alignment with the Bargon criteria. A total of 67 patients met the study inclusion criteria; 20 cases in the AP screw, 13 cases in the PA screw and 34 cases in the plate group. The mean follow-up period was 14.4 +/- 2.23 months in AP, 16.3 +/- 2.56 months in PA and 17.1 +/- 3.01 months in the plate group. Better AOFAS scores were obtained in the PA group and the plate group compared to the AP screw group (p < 0.001). No statistically significant difference was found between the groups in respect of VAS scores during walking and dorsiflexion restriction. Better radiological reduction was observed in the PA screw group and the plate group (p < 0.001). Hydrolysis of arabinoxylan (AX) by glycoside hydrolase family 10 (GH10) xylanases produces xylo-and arabinoxylo-oligosaccharides ((A)XOS) which have shown prebiotic effects. The thermostable GH10 xylanase RmXyn10A has shown great potential to produce (A)XOS. In this study, the structure of RmXynlOA was investigated, the catalytic module by homology modelling and site-directed mutagenesis and the arrangement of its five domains by small-angle X-ray scattering (SAXS). Substrate specificity was explored in silico by manual docking and molecular dynamic simulations. It has been shown in the literature that the glycone subsites of GH10 x . . .ylanases are well conserved and our results suggest that RmXyn10A is no exception. The aglycone subsites are less investigated, and the modelled structure of RmXyn10A suggests that loop alpha(6)beta(6) in the aglycone part of the active site contains a non-conserved alpha-helix, which blocks the otherwise conserved space of subsite + 2. This structural feature has only been observed for one other GH10 xylanase. In RmXyn10A, docking revealed two alternative binding regions, one on either side of the alpha-helix. However, only one was able to accommodate arabinose-substitutions and the mutation study suggests that the same region is responsible for binding XOS. Several non-conserved structural features are most likely to be responsible for providing affinity for arabinose-substitutions in subsites +1 and + 2. Our objective in this study was to assess the changes in medial gastrocnemius muscle (GCM) stiffness after botulinum toxin A (BTA) injection in children with cerebral palsy (CP) by using acoustic radiation force impulse (ARFI) elastography and to research the usability of this technique in clinical practice. Twenty-four spastic lower extremities of 12 children with CP were assessed. BTA injection treatment was applied to the medial GCM. Muscle stiffness was measured with the ARFI technique before the procedure and a month after the procedure. Shear wave velocity (SWV) values and MAS scores before and after the treatment were compared. Mean SWV values were measured as 3.20 +/- 0.14 m/s before BTA and as 2.45 +/- 0.21 m/s after BTA, and the difference between them was found to be statistically significant (p < 0.001). Mean MAS score (2.33 +/- 0.70) after BTA decreased significantly when compared to the score before BTA (2.96 +/- 0.62) (p = 0.001). SWV values positively correlated with MAS scores (rho = 0.578, p = 0.003). The interobserver agreement expressed as interclass correlation coefficient (ICC) was 0.65 (95% CI 0.33-0.84, p < 0.001). Epiploic appendagitis, caused by inflammation of small adipose tissue on the colon wall, is a rare cause of acute abdominal pain in the pediatric population. It is nearly impossible to establish a specific diagnosis merely on the basis of clinical findings; thus, radiological evaluation is always necessary. In this report, we present the cases of three children with abdominal pain who were diagnosed with epiploic appendagitis. All cases were successfully treated with conservative management.

Hantaviruses infect humans via inhalation of viral particles within secretions of infected rodents or rarely through direct contact with infected rodents. Determining the prevalence of hantavirus infections among rodent populations is of vital importance to obtain information on hantavirus-related cases and to predict possible outbreaks. We hypothesized that DOBV strains circulating in the Thrace Region in Turkey would be related to other Balkan DOBV strains. In this study, hantavirus infections in the rodent population of the Krklareli-neada Region (north-western Turkey, near the Bulgarian border) were investigated. The aim of this study was to evaluate and compare the effects of remote and direct ischaemic preconditioning (RIPC and DIPC) histopathologically in the rat renal IR injury model. A dip-coating technique was applied to prepare a selective membrane on a commercial ceramic mesoporous support. Single gas components used for permeance and selectivity were CH4, CO2, H-2, He, N-2, and O-2 (BOC UK) with at least 99.999 (% v/v) purity. The permeances and selectivities were obtained at room temperature and transmembrane pressure differences between 0.05 up to 5.0 barg. Gas permeation experiments showed the permeance of CO2 to be strongly influenced by surface diffusion mechanism. Objective: Bupivacaine, a local anaesthetic substance, is used as a regional-anaesthesia agent. Lidocaine, a sodium channel blocker, is used in combination with epinephrine for regional anaesthesia. We aimed to evaluate the effects of lidocaine with epinephrine (LE) at different doses on bupivacaine-induced cardiotoxicity in rats. Methods: In our study, 24 Wistar albino rats were divided into four groups: I) Control; II) LE, 1 mg kg(-1); III) LE, 3 mg kg(-1) and IV) LE, 6 mg kg(-1). Intravenous bupivacaine was administered at a dose of 3 mg kg(-1) min(-1) to the anaesthetized rats in all groups until cardiac asystole was achieved.
