The role of patient's systemic characteristics and plateletcrit in developing toxic anterior segment syndrome after uneventful phaco surgery: A case-control study

Purpose To compare the systemic and ocular characteristics and laboratory findings of patients developing toxic anterior segment syndrome (TASS) after uneventful phaco surgery with unaffected subjects undergoing the same surgery in the same session. Design A retrospective case-control study. Methods The study group consisted of 26 eyes of 26 patients who underwent uneventful phaco surgery and who went on to develop TASS, while the control group included 39 subjects who had routine phaco surgery in the same session by the same surgeon. The preoperative systemic diseases, complete blood count parameters, glycosylated hemoglobin (HbA1c), biochemical parameters, thyroid hormone profiles, and the surgical features were compared between the two groups. Results Type 2 diabetes mellitus (DM), systemic hypertension (HT), hyperlipidemia, chronic ischaemic heart disease, and chronic renal failure were significantly more common in the TASS group (p < 0.05). Proliferative diabetic retinopathy was also more frequent in the TASS group (p = 0.003). Mean HbA1c% values, white blood cell count, neutrophil/lymphocyte ratio, platelet counts, platelet distribution width, and plateletcrit parameters were significantly higher in the TASS group (p < 0.05). Multivariate logistic regression analysis revealed that a high plateletcrit level (p = 0.001, odds ratio [95% CI]; 22.27 [3.36-147.76]) and systemic HT (p = 0.044, odds ratio [95% CI]; 7.13 [1.05-48.12]) are independently associated with the development of TASS. Conclusion Although TASS may arise as a result of insufficient sterilization of instruments or intraocular solutions, patient factors may also contribute to its development. Systemic vascular disorders such as uncontrolled type 2 DM, systemic hypertension, and hyperlipidemia may increase the risk of TASS after uneventful phaco surgery. Abnormal parameters associated with systemic inflammation, such as higher plateletcrit level, may facilitate the development of TASS. Refractive disorders are common health problems in the community and they are the most important cause of visual impairment. In this study, it was aimed to classify the individuals who have hypermetropia and myopia refractive disorders or not. Impact of hybrid neural network on the early diagnosis of diabetic retinopathy disease from video-oculography signals

In this study, we introduce two hybrid artificial neural network models with particle swarm optimization algorithm to diagnose diabetic retinopathy based on the Video-Oculography signals. The hybrid models use Discrete Wavelet Transform and Hilbert-Huang Transform separately to extract features from the signals. The classification performance of both models is analyzed comparatively. We show that the model based on Hilbert–Huang Transform exhibits better classification performance than the model based on the Discrete Wavelet Transform. © 2018 Elsevier Ltd

Comparison between clinical outcomes of two different designs of toric multifocal intraocular lenses

To compare the clinical outcomes of plate haptic and open-loop haptic toric multifocal intraocular lenses (IOLs). In this retrospective, comparative clinical trial, two different designs of multifocal toric IOL were implanted in 49 eyes of 38 cases with corneal astigmatism ? 0.75 Diopter (D). The cases that underwent AcrySof IQ restore multifocal toric IOL (Alcon, open-loop-haptic) implantation were assigned to Group1 (n=19) and the cases that underwent Acriva Reviol multifocal toric IOL (VSY, plate-haptic) implantation were assigned to Group 2 (n=30). After surgery, the groups were compared in terms of degree of IOL rotation, resid . . .ual spherical refraction and astigmatism and uncorrected near and distance visual acuity. With regard to the results of postoperative 1st month, 3rd month, 6th month and 1st year, no difference was determined in terms of uncorrected distance visual acuity, spherical equivalent (SE), degree of astigmatism (CYL), degree of rotation, and uncorrected near visual acuity (p>0.05). Significant increase was determined in all postoperative follow-up periods in terms of the distance and near visual acuity as compared to preoperative values (p0.05). Clinical outcomes of plate haptic Acriva Reviol multifocal toric IOL and open-loop haptic Acrysof IQ multifocal toric IOL are similar. Maxillary Sinus Mucocele with Exudative Retinal Detachment and Ocular Dystopia Without Diplopia

Mucocele is a cystic lesion occuring by the obstruction of drainage of normal paranasal sinus secretions due to infections, trauma, and rarely tumor. This lesion can impress to orbital contents by eroding orbital bone and it can be confused with malignant tumors. Most mucoceles originate from frontal and ethmoid sinuses, whereas maxillary and sphenoid sinus presentation is rare. In this case report we have presented a maxillary sinus mucocele case leading to asymptomatic inferior exudative retinal detachment and pushing up glob. A 43-year-old woman presented with swelling on her right malar region and lower eyelid lasting one year a . . .nd increasing last three months. The patient had upward ocular dystopia and an exudative retinal detachment not extending the macula at the inferior retinal area in her right eye. She had diagnosed right maxillary sinus tumor suspicion by clinical examination and imaging methods. Operation was performed by the earnose-throat department. While ocular dystopia completely recovers, exudative retinal detachment was still present at the postoperative first day. As a result, maxillary sinus mucocele can lead to findings such as ocular dystopia without diplopia and exudative retinal detachment, and clinical signs may mimic the tumor. Mukosel travma, infeksiyon ve nadiren de tümör nedeniyle normal paranasal sinus drenajının engellenmesi ile oluşan kistik bir lezyondur. Bu lezyon orbital kemiği erode ederek orbital içeriğe bası yapabilir ve malign tümörlerle karıştırılabilir. Çoğu mukosel frontal ve etmoid sinus kaynaklıdır, oysa maksiller ve sfenoidal sinus presentasyonu nadirdir. Biz bu makalede globu iten ve asemptomatik alt kadran eksudatif retina dekolmanına yol açan bir maksiller sinus mukoseli olgusunu sunduk. Kırk-üç yaşında bir kadın, sağ yanak ve alt göz kapağı bölgesinde 1 yıldır var olan ve son 3 aydır artan şişlik ile başvurdu. Glopta yukarı distopi ve retinanın alt bölgesinde makülaya taşmayan eksudatif retina dekolmanı mevcuttu. Klinik muayene ve görüntüleme yöntemleri ile maksiller sinüs tümörü şüphesi ile kulak-burun-boğaz hastalıkları bölümü tarafından operasyon uygulandı. Postoperatif birinci gün oküler distopi tamamen düzelirken retinal eksudasyon hala mevcuttu. Sonuç olarak, maksiller sinus mukoseli diplopi olmaksızın oküler distopi ve eksudatif retina dekolmanı gibi bulgulara yol açabilir ve klinik bulgular tümörü taklit edebilir Daha fazlası Daha az
